Özet

Sistemik kanser, kafatası metastazı veya yeni nörolojik belirti varlığında tümör dura boyunca ya da BOS içeren leptomeningeal yüzeylere yayılabilir. Kontrastlı MR dural kalınlaşma ve boyanmayı, BT ise komşu kalvaryumdaki düzensizlik ve kemik yıkımını gösterebilir. Dural ve leptomeningeal tutulumun görüntüleme bulguları örtüşebilir; tutulum bölmesinin kesin ayrımı her zaman görüntülemeyle yapılamaz ve BOS incelemesi veya biyopsi tanıyı destekleyebilir. Meneninks metastazı şüphesinde kontrastlı MR’da multifokal nodüler meningeal boyanma ve düzensiz dural kalınlaşma görülebilir.

Okuma sırası

  1. Konveksite durası. Kontrastlı görüntülemelerde dural tabanlı ekstraaksiyel lezyon ve eşlik eden dural kalınlaşma değerlendirilir.
  2. Komşu kalvaryum. Dural lezyona komşu kalvaryumda kemik düzensizliği veya osteoliz; MR’da kemik iliği ve diploik alan tutulumu değerlendirilebilir.
  3. Kortikal sulkuslar. Dural kalınlaşma ve boyanmayla birlikte sulkuslar, sisternler veya fissürler boyunca leptomeningeal boyanma izlenebilir.
  4. Ventriküller ve perilezyonel parankim. Leptomeningeal yayılıma bağlı BOS dolaşım bozukluğu ve hidrosefali olasılığı için ventrikül boyutları değerlendirilir.

Faz ve pencere

Kontrastsız
Kafatası kemiğinde osteoliz veya düzensizlik ve eşlik eden ekstraaksiyel yumuşak doku lezyonu değerlendirilir. Leptomeningeal metastaz şüphesinde MR, tercih edilen görüntüleme yöntemidir.
Kontrastlı tanısal
Kontrastlı MR’da düzensiz dural kalınlaşma ve multifokal nodüler meningeal boyanma; leptomeningeal yayılımda sulkus ve sistern boyanması, eşlik eden hidrosefaliyle birlikte görülebilir. Görüntüleme çalışmaları negatif olsa bile hastalık dışlanamaz; leptomeningeal metastazın tespitinde manyetik rezonans görüntüleme (MR) tercih edilen yöntemdir.
Kemik algoritması
Dural lezyon komşuluğunda kalvaryal osteoliz ve kemik iliği/diploik alan tutulumu görülebilir; bu bulgular kemikten doğrudan yayılım olasılığını düşündürebilir.

Önerilen pencereler: Beyin (G 80 / M 40), Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Fokal dural nodül — iç tabulaya dayanan ekstraaksiyel yumuşak doku kalınlaşması kontrast sonrası belirginleşebilir.
  • Yaygın pakimeningeal kalınlaşma — dura boyunca plak veya düzensiz çizgisel kontrastlanma görülür; subaraknoid sulkuslara uzanım şart değildir.
  • Leptomeningeal/sulkusal boyanma — kortikal sulkusların, fissürlerin veya bazal sisternlerin yüzeyini izleyen ince ya da nodüler kontrastlanma.
  • Kemik tutulumuyla ilişkili ekstraaksiyel lezyonlarda kalvaryal osteoliz veya kemik iliği/diploik alan tutulumu eşlik edebilir.
  • Dural kitle etkisi — komşu kortekste bası, sulkus silinmesi ve vazojenik ödem gelişebilir.
  • Leptomeningeal metastaz BOS dolaşımını bozarak intrakraniyal basınç artışına ve hidrosefaliye yol açabilir; görüntülemede ventrikül genişlemesi görülebilir.
  • Eşlik eden parankimal metastaz — kontrastlanan kitle ve çevresinde geniş ödemiyle ortaya çıkabilir; sistemik metastaz varlığını destekler.

Normalde

Meneningeal patolojilerde dura veya leptomeninkslerde kalınlaşma ve anormal kontrastlanma görülebilir. Dural lezyon değerlendirilirken ekstraaksiyel yerleşim ile komşu kalvaryumda kemik değişikliği ve ekstrakraniyal uzanım olup olmadığı incelenir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Menenjiyom
homojen kontrastlanma ve komşu kemik tutulumuyla dural metastaza benzer radyolojik bulgular verebilir; bu iki yapıyı görüntüleme ile ayırt etmek zordur.
Dural lenfomalar menenjiyom ve dural metastazlarla radyolojik olarak örtüşebilir; tanıda biyopsi ve hematolojik evreleme gerekebilir.
Enfeksiyöz pakimenenjit
dura boyunca yaygın kalınlaşma ve kontrastlanma yapabilir; komşu sinüs, mastoid veya kemik enfeksiyonu araştırılır.

Tuzaklar

  • Negatif görüntüleme leptomeningeal metastazı dışlamaz; klinik şüphe sürerse BOS incelemesi ve tekrarlanan görüntüleme gerekebilir.
  • Dural bazlı metastazlar radyolojik olarak menenjiyomları taklit edebilir ve bu durum tanısal zorluk yaratabilir.
  • Leptomeningeal boyanmanın lineer veya nodüler paternleri özgül değildir ve neoplastik, enfeksiyöz ya da inflamatuvar nedenlerde görülebilir; dağılımı ve eşlik eden bulgular ayırıcı tanıya katkı sağlar.
