Özet

Pineal bölge kitlesi, derin venler, tektum ve akuadukt çevresindeki dar anatomik alanda büyüyerek BOS akımını bozabilen farklı tümör ve kistleri kapsar. Kontrastsız BT özellikle kitle etkisini, kalsifikasyonun pineal bezle ilişkisini, kanamayı ve obstrüktif hidrosefaliyi hızlı gösterir. Konvansiyonel BT ve MR görüntüleme özellikleri sıkça örtüşür; bu nedenle görüntüleme tek başına güvenilir tanı koydurucu değildir ve doku tanısı gerekebilir. Akuadukt tıkanıklığı ve akut hidrosefali gözden kaçarsa klinik kötüleşme riski taşıyan basınç artışı fark edilmeyebilir.

Kontrastsız tanısal
Pineal bez/kuadrigeminal sistern düzeyinde kitle, normal pineal kalsifikasyonun kitle içinde kalması ya da yer değiştirmesi, olası kanama ve akuadukt önündeki bası değerlendirilir. Üçüncü ve lateral ventrikül genişlemesi ile periventriküler düşük atenüasyon aynı incelemede aranır; BT kalsifikasyon ve hidrosefali için tanısaldır, tümör alt tipi için değildir.

Önerilen pencereler: Beyin (G 80 / M 40), Kemik (G 1800 / M 400).

BT bulguları

  • Pineal/kuadrigeminal sistern yumuşak doku kitlesi — tektumun üstünde, üçüncü ventrikül arkasında ve derin venlerin komşuluğunda yer alır.
  • Pineal kalsifikasyonun kitle içindeki konumu — bez kalsifikasyonu tümör tarafından çevrelenmiş veya kitle içinde hapsolmuş görünebilir.
  • Pineal kalsifikasyon — Pineal bölge kitlelerinde değişken kalsifikasyon görülebilir ve görüntüleme özellikleri tümörler arasında örtüşebilir.
  • Akuadukt ve tektum basısı — posterior üçüncü ventrikül çıkışında daralma veya dorsal orta beyin konturunda bası.
  • Akuadukt basısına bağlı obstrüktif hidrosefalide lateral ve üçüncü ventriküller genişleyebilir.
  • Transependimal ödem — frontal horn çevresinde periventriküler hipodensite artmış BOS basıncını destekler.
  • Pineal bölge kitlesinde BT kalsifikasyonu veya kanamayı gösterebilir; tümörlerin görüntüleme bulguları örtüşebildiğinden görüntüleme tek başına bazı olgularda tanısal güven sağlamayabilir.

Normalde

Pineal bölge kitlesinde kalsifikasyon görülebilir; değişken kalsifikasyon, tümörler arasında örtüşebilen görüntüleme özelliklerinden biridir. Tektum ve akuaduktın kitleyle ilişkisini ve ventrikül boyutlarını değerlendirin; pineal bölge kitlesi akuaduktı daraltarak obstrüktif hidrosefaliye yol açabilir. Pineal bezde fizyolojik kalsifikasyon görülebilir; pineal bölge tümörlerinde kalsifikasyon değişken olabilir ve tümörler arasında görüntüleme özellikleri örtüşebilir, bu nedenle kalsifikasyon tek başına tümör alt tipini belirlemez.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Pineal kist
MR incelemelerinde sıklıkla tesadüfen saptanan, çoğunlukla asemptomatik kistik oluşumlardır; apopleksi veya hidrosefali nadir komplikasyonlardır ve klinik kötüleşme ya da belirgin BOS yolu tıkanıklığı varsa acil değerlendirme gerekir.
Germ hücreli tümör
kalsifikasyon içeren pineal kitle görülebilir; görüntüleme özellikleri örtüşebildiğinden kalsifikasyon tek başına histolojik alt tipi belirlemez.
Pineal parankimal tümör
konvansiyonel BT ve MR bulguları pineal bölge tümörleri arasında örtüşebildiğinden, görüntüleme tek başına bazı olgularda tanısal güven sağlamayabilir.
Tektal gliom
pineal bez merkezinden ziyade dorsal orta beyin/tektumda genişleme yapar; akuadukt daralması eşlik edebilir.
Kuadrigeminal sistern epidermoid kisti
sistern düzeyinde orta hat lezyonu oluşturabilir; akuadukta bası eşlik ederek obstrüktif hidrosefaliye yol açabilir.
Derin ven trombozu ayırıcı tanısı
Pineal bölge tümörlerini çevreleyen başlıca venöz yapılar internal serebral venler, Galen veni ve Rosenthal bazal venleridir; tümörün bu yapılarla ilişkisi cerrahi yaklaşım açısından değerlendirilir.

Tuzaklar

  • Pineal kalsifikasyon varlığını tek başına tümör diye yorumlama; normal pineal kalsifikasyon yaygındır, bezin kitleyle ilişkisini ve yer değiştirmeyi değerlendir.
  • Görüntülemede görülen kalsifikasyon ve diğer morfolojik özellikler tümör tipleri arasında önemli ölçüde örtüşebilir; bu nedenle kesin alt tip tayini için histopatoloji esastır.
  • Akuadukt basısını yalnız kitle çapına bakarak dışlama; ince kesitlerde daralma ve yukarı akım ventrikül genişlemesini birlikte ara.
  • BT'de kitle görünümünü benign/malign sınıflaması olarak sunma; tümör alt tipi için MR ve gerektiğinde doku tanısı gerekir.
