Özet

Üçüncü ventrikül koloid kisti, çoğunlukla foramen Monro komşuluğunda bulunan ve BOS akımını iki tarafta birden kesebilen benign bir epitel kistidir. Kontrastsız BT'de ön üçüncü ventrikülde yuvarlak lezyon, lateral ventrikül genişlemesi ve akut basınç bulguları aranır; kistin atenüasyonu değişken olduğundan izodens olması tanıyı dışlamaz. BT akut obstrüktif hidrosefaliyi ve foramen Monro düzeyindeki koloid kisti gösterebilir, ancak koloid kistler BT ve MR incelemelerinde gözden kaçabilir; MR kistin sinyal özelliklerini ve içeriğini daha iyi karakterize eder. Obstrüksiyon ve ventrikül genişlemesinin gözden kaçması hızla kötüleşebilen kafa içi basıncın tanınmasını geciktirebilir.

Faz ve pencere

Kontrastsız tanısal
Ön üçüncü ventrikül tavanında/foramen Monro düzeyinde iyi sınırlı yuvarlak veya oval lezyon aranır; çoğu gri cevhere göre hiperdens olsa da izodens ya da hipodens olabilir. Aynı seride lateral ventriküllerin genişlemesi, temporal hornların belirginliği, periventriküler transependimal ödem ve sulkus basısı değerlendirilir; akut tanısal amaç için kontrastsız seri yeterlidir.

Önerilen pencereler: Beyin (G 80 / M 40).

BT bulguları

  • Ön üçüncü ventrikül odağı — foramen Monro'ya komşu, orta hatta yuvarlak veya oval, iyi sınırlı kistik lezyon.
  • Değişken atenüasyon — kistler sıklıkla beyin dokusundan hiperdens veya izodens görünebilir.
  • Foramen Monro düzeyinde BOS akım kesilmesi — küçük lezyon bile dar geçişi tıkayarak obstrüksiyon oluşturabilir.
  • Lateral ventrikül genişlemesi — iki foramenin etkilenmesine bağlı bilateral ventrikülomegali; başlangıçta asimetrik olabilir.
  • Foramen Monro düzeyindeki obstrüksiyon, lateral ventriküllerden BOS çıkışını engelleyerek bu ventriküllerde genişlemeye yol açabilir.
  • Transependimal BOS geçişi — frontal horn çevresinde periventriküler düşük atenüasyon akut basınç artışını destekler.
  • İleri kitle etkisi — sulkus silinmesi, bazal sistern daralması veya orta hat yapılarında bası acil önem taşıyan eşlikçi bulgulardır.

Normalde

Normal foramen Monrolar, lateral ventriküllerin gövdelerini üçüncü ventriküle bağlayan açık BOS geçişleridir; ön üçüncü ventrikül tavanında ayrı bir yuvarlak yumuşak doku odağı bulunmaz. Ventrikülleri aynı kesit düzeyinde ve simetrik kıyaslayın: normalde lateral ventrikül gövdeleri geniş değil, periventriküler beyaz cevher de hipodens değildir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Santral nörositom
genellikle lateral ventrikül içinde, septum pellucidum yakınında daha geniş kitle yapar; kalsifikasyon ve heterojenlik gösterebilir.
Subependimom
ventrikül duvarına oturan yavaş büyüyen kitle olup foramen Monro önünde tipik koloid kist konumunu paylaşmaz.
Dev hücreli astrositom
sıklıkla foramen Monro çevresinde, tuberoz skleroz zemininde ve düzensiz/heterojen kitle şeklindedir.
İntraventriküler kan pıhtısı ayırıcı tanıda düşünülmelidir; koloid kist içinde de nadiren kanama gelişebilir ve akut hacim artışıyla hidrosefaliye yol açabilir.
Diğer intraventriküler kistler (araknoid, epidermoid) ayırıcı tanıda yer alır; üçüncü ventrikül araknoid kisti nadirdir, koloid kist ise tipik olarak foramen Monro yakınında yerleşir ve T1 hiperintens, T2 hipointens sinyal gösterebilir.
Kordoid gliom, ön üçüncü ventrikül veya suprasellar bölgede görülebilen bir tümördür; kontrastlı MR'da homojen kontrastlanma gösterebilir ve üçüncü ventrikül tümörlerinin ayırıcı tanısında düşünülmelidir.

Tuzaklar

  • Kisti yalnız hiperdens görünüm üzerinden aramayın; koloid kistler izodens olarak da görünebilir ve obstrüksiyon görüntüde ince kalabilir.
  • Ventrikül asimetrisini zararsız varyant kabul etmeden önce foramen Monro ve ön üçüncü ventrikül tavanını incele.
  • Lateral ventrikül genişlemesini tek başına koloid kiste bağlama; lezyonun foramen düzeyindeki tıkanıklığı ve üçüncü/dördüncü ventrikül örüntüsüyle uyumunu ara.
  • BT'de kist seçilemiyor diye klinik açıdan anlamlı foramen Monro obstrüksiyonunu dışlama; yüksek şüphede MR daha iyi lezyon gösterimi sağlayabilir.
