Özet

Baş ağrısı, kusma, bilinç değişikliği veya fokal bulguyla başvuran hastada, bir kitle BOS akımını daraltıp hızla basınç artışına yol açabilir. Acil kontrastsız beyin BT, ventrikül genişlemesini, olası tıkanma düzeyini ve eşlik eden kitle etkisini dakikalar içinde gösterir; küçük ya da izodens tıkayıcı lezyonun karakterizasyonu için MRG gerekebilir. Proksimal ventriküllerin genişleyip tıkanmanın distalindeki boşlukların görece küçük kalması obstrüksiyon yerini bulmaya yardım eder. Transependimal ödem, bazal sisternlerin silinmesi veya herniasyon bulgusu gözden kaçarsa akut nörolojik kötüleşme ve ölümcül basınç etkisi tanınmayabilir.

Faz ve pencere

Kontrastsız tanısal
Ventriküller ve BOS boşlukları hipodens görünür; tıkanma seviyesinin proksimalindeki ventriküllerin genişlemesi, periventriküler beyaz cevherde silik hipodens bantlar ve olası hiperdens/izodens intraventriküler ya da komşu kitle aranır.

Önerilen pencereler: Beyin (G 80 / M 40), Subdural (G 200 / M 75).

BT bulguları

  • Tıkanmanın üst tarafındaki ventrikül bölümleri geniş, altındaki bölümler görece normal veya küçüktür.
  • Foramen Monro tıkanırsa bir ya da iki lateral ventrikül genişler; üçüncü ve dördüncü ventriküller genişlemeyebilir.
  • Akuadukt düzeyindeki engelde lateral ve üçüncü ventriküller genişlerken dördüncü ventrikül görece korunur.
  • Dördüncü ventrikül çıkışındaki engelde dördüncü ventrikül ile supratentoryal ventriküller birlikte genişleyebilir.
  • Periventriküler hipodensite, özellikle frontal ve oksipital boynuz çevresinde, transependimal BOS geçişiyle uyumlu interstisyel ödemi düşündürür.
  • Kolloid kist, pineal bölge kitlesi veya arka çukur kitlesi BOS akımını tıkayarak obstrüktif hidrosefaliye neden olabilir.
  • İleri basınçta temporal boynuzlar belirginleşir, sulkuslar ve bazal sisternler silinir; orta hat kayması veya aşağı/yukarı herniasyon eşlik edebilir.

Normalde

Kıyaslamada yalnız toplam genişliğe değil, hangi ventrikül segmentinin büyüdüğüne, hangi BOS geçiş düzeyinde daralma olduğuna ve periventriküler beyaz cevherin normal keskin sınırının silinip silinmediğine bakılır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Transependimal BOS akımı veya periventriküler hipodensite, ventrikül genişlemesi ve şunt gereksinimi ile güçlü şekilde ilişkili olabilir.
  • Yaşlı hastada periventriküler kronik küçük damar hastalığı hipodensitesi transependimal akımı taklit eder; ventrikül şekli ve seri görüntülerle birlikte okunur.
  • Kolloid kistler üçüncü ventrikül ön kısmında yer alabilir ve BOS yolunu tıkayarak obstrüktif hidrosefaliye yol açabilir.

    Akuadukt obstrüksiyonu. Akuadukt tıkanınca lateral ve üçüncü ventriküller genişleyip dördüncü ventrikül distal kaldığı için korunabilir. Komünikan hidrosefalide fokal geçiş engeli beklenmez; ex vacuo durumda sulkuslar da genişler; normal asimetri transependimal ödemle açıklanmaz.

  2. Geniş ventriküllerin çevresinde yeni hipodens beyaz cevher ve silik bazal sisternler var. Bu kombinasyon neyi gösterir?
