Özet

Kemik metastazı taşıyan femur, humerus ve vertebra gibi yük aktaran kemiklerde tümörün yapısal zayıflatması kırığa veya omurga instabilitesine yol açabilir. BT ince kesit ve çok düzlemli rekonstrüksiyonlarla kortikal çevreyi, kırık hattını, fragman deplasmanını ve vertebra hizalanmasını ortaya koyar. Uzun kemik kırık riski Mirels gibi klinik-radyografik puanlarla, spinal instabilite ise SINS gibi ayrı bir sistemle ele alınır; BT bu araçların görüntüleme bileşenlerini sağlar fakat ağrı ve işlev öyküsünün yerini tutmaz. Kırığın ya da kanal yönüne yer değiştiren parçanın gözden kaçması hareket kaybı, nörolojik zarar ve bakımın gecikmesiyle sonuçlanabilir.

Kontrastsız tanısal
BT, kortikal kemiği, arka duvar bütünlüğündeki bozulmayı ve kırık morfolojisini değerlendirmede yararlıdır. İnce kesit BT ve çok düzlemli rekonstrüksiyonlar, kortikal süreksizlik, arka duvar tutulumu ve retropulsyonu cerrahi planlama bağlamında ayrıntılı olarak gösterir.

BT bulguları

  • Metastatik litik kemik lezyonunda kortikal yıkım görülebilir.
  • Keskin çizgisel lusensi ve kortikal basamaklanma, tümörlü kemikten geçen kırık hattını gösterebilir.
  • Fragmanların açılanması, kısalması veya translasyonu kırığın deplasmanını ve anatomik hizanın bozulmasını tanımlar.
  • Femur proksimali ya da diafizindeki litik lezyonun korteks boyunca uzanımı, kırık hattı görülmese bile mekanik risk değerlendirmesini gerektirir.
  • Vertebra korpus yüksekliğinin azalmasıyla birlikte arka duvar konturunun bozulması, patolojik çökme ve kanal yönünde parça hareketi olasılığını artırır.
  • Metastatik kemik lezyonunda kortikal yıkıma eşlik eden yumuşak doku uzanımı görülebilir.

Mirels uzun kemik kırık riski puanı
Mirels puanlamasında ağrı, anatomik yerleşim, lezyon boyutu ve radyografik görünüm dikkate alınır. Mirels puanında ≥9 geleneksel olarak alt ekstremite metastazlarında yaklaşan patolojik kırık eşiği kabul edilir; humerus metastazlarında ≥8 daha iyi öngörü profili göstermiştir. Skor klinik bileşen içerir.
Spinal Instability Neoplastic Score (SINS)
Radyoterapi uygulanmış hareketli omurga metastazlarında SINS ≥7, artmış patolojik kırık olasılığıyla ilişkili bulunmuştur.
Mirels puanı
Mirels puanlamasında ağrı, anatomik yerleşim, lezyon boyutu ve radyografik görünüm birlikte değerlendirilir. Skor klinik ağrı bilgisini içerir ve tek başına BT ölçümü değildir.
Spinal Instability Neoplastic Score (SINS)
SINS, spinal metastazı olan hastalarda omurga stabilitesini standart biçimde değerlendirmek için kullanılır; radyoterapi uygulanmış hareketli omurga metastazlarında SINS ≥7, artmış patolojik kırık olasılığıyla ilişkili bulunmuştur.

Normalde

BT, kortikal kemiğin, arka duvar tutulumunun ve kırık morfolojisinin değerlendirilmesinde özellikle instabilite şüphesinde veya cerrahi planlamada yararlıdır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Osteoporotik vertebra kırığında son plak kırığı, vertebra şekil değişikliğinden daha temel bir radyolojik göstergedir.
Akut travmatik vertebra kırıkları motorlu araç çarpışması veya yüksekten düşme gibi travmalarla ilişkili olabilir; malignite ilişkili kırıklarda ise tümör infiltrasyonu vertebranın yapısal bütünlüğünü bozabilir.
Vertebral hemanjiom
kalınlaşmış dikey trabeküller aksiyel kesitte noktalı, sagittal kesitte çizgili görünüm verir; posterior duvar kırığına bağlı yumuşak doku kitlesinden ayrılır.

  • Tam kırık hattı görülmemesi uzun kemikte kırık riskini sıfırlamaz; lezyonun korteksle çevresel ilişkisini ve kemik çapındaki yayılımını ortogonal planlarda inceleyin.
