Özet

Metastatik meme kanserinin %65-75'inde ve ileri evre prostat kanserinin %80'inden fazlasında kemik metastazı görülür; ağrı ve patolojik kırıklar gibi iskelet ilişkili olaylara yol açar. BT kemik matriksini değerlendirmede tercih edilir; kemik iliğinin değerlendirilmesinde MRG en duyarlı görüntüleme yöntemidir. Litik, sklerotik ve mikst görünüm primer tümörün biyolojisiyle ilişkili olsa da tek başına tümör kökenini kanıtlamaz. Lezyonun yük taşıyan kemikte veya vertebrada oluşturduğu yapısal zayıflığı atlamak patolojik kırık ya da nörolojik komplikasyonun geç fark edilmesine yol açabilir.

Kontrastsız tanısal
Kemik algoritmalı kontrastsız BT'de litik odak trabeküler yoğunluk kaybı ve düzensiz kemik yıkımı, sklerotik odak çevre medüller kemiğe göre artmış yoğunluk, mikst odak ise aynı lezyonda iki patern olarak izlenir. Kortikal kesinti ve kemik dışına uzanan yumuşak doku kitlesi yapısal yayılımı gösterir.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Litik metastazda medüller trabeküller seyrelir veya silinir; sınır çoğu kez coğrafi, güve yeniği ya da permeatif yıkım görünümündedir.
  • Sklerotik metastaz, vertebra korpusu, kaburga veya pelvis medüllasında yamalı ya da birleşen kemik yoğunluğu artışı yapar.
  • Mikst metastazda litik merkez ile çevresel veya odaksal osteoskleroz aynı kemik odağında birlikte bulunur.
  • Kortikal incelme, oyulma ya da tam kat kesinti; lezyonun kemiğin mekanik kabuğunu zayıflattığını gösterir.
  • Periost boyunca düzensiz yeni kemik ve kemik dışı yumuşak doku kitlesi, basit sklerotik odaktan daha agresif kemik tutulumunu düşündürür.
  • Vertebral metastazlarda posterior duvar yıkımı patolojik kırıkla birlikte görülebilir; spinal instabilite ve metastatik spinal kord kompresyonu olası komplikasyonlardır.

Ölçütler ve sınıflamalar

BT morfolojisine göre kemik metastazı
Osteolitik patern kemik yıkımının baskın olduğu; osteoblastik/sklerotik patern yeni kemik yapımının baskın olduğu; mikst patern ise ikisinin aynı lezyonda birlikte izlendiği gruptur. Bu morfolojik ayrım primer tümörü tek başına belirlemez.

Normalde

Aynı seviyedeki normal vertebra korpusunda trabeküler ağ homojen dağılır, kortikal çevre kesintisizdir ve yükseklik korunur; normal iliak kanat ve kaburgada odaksal destrüksiyon ya da yamalı skleroz görülmez. Şüpheli odağı karşı tarafın eş kemik bölgesiyle kıyaslayarak medüller yoğunluk, kortikal kalınlık ve dış kontur farklılığını ayırt edin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Paget hastalığı
kemik genişlemesi, kortikal kalınlaşma ve kaba trabeküller odaksal metastazdan farklı bir yeniden yapılanma paterni oluşturur.
Osteoporotik vertebra kırığı
end-plaka kırığının varlığı temel göstergedir; deformite olmaksızın bile korteks kırığı görülebilir.
Osteomiyelit
spondilodiskitte enfeksiyon intervertebral diski ve komşu vertebraları tutar.
Benign kemik adacıklarıyla seyreden osteopoikiloz
özellikle periartiküler uzun kemik ve pelviste benzer boyutlu, düzenli dağılımlı sklerotik odaklar yapar.

Tuzaklar

  • Tedavi sonrası skleroz, önceden bilinen litik metastazın iyileşme yanıtı olabilir; yeni bir sklerotik odak kararını önceki inceleme ve tümör yüküyle birlikte verin.
  • Diğer görüntülemelerde şüpheli olup BT’de belirgin karşılığı görülmeyen kemik lezyonlarında, kemik iliği ödemi haritalaması patolojik alanı gösterebilir; bu nedenle BT’de belirgin odak görülmemesi kemik iliği tutulumunu kesin olarak dışlamaz.
  • HU ölçümü litik ve sklerotik kemik lezyonlarını ayırt etmeye katkı sağlayabilir; lezyonun sınırı, dağılımı ve diğer görüntüleme özellikleri de değerlendirilmelidir.
