Özet

Nekrotizan enterokolit (NEK), özellikle prematüre ve ağır hasta yenidoğanlarda bağırsak duvarını etkileyen, hızla ilerleyebilen bir acildir. Şüpheli NEK’te tanı ve seri takip çoğunlukla karın radyografisi ile ultrasona dayanır; BT rutin başlangıç incelemesi değildir. BT başka bir klinik gerekçeyle elde edilmişse duvar içi gazı, portal venöz gazı, duvar perfüzyonunu ve perforasyon bulgularını aynı anatomik haritada gösterebilir. Duvar nekrozu ya da perforasyonun fark edilmemesi ağır sistemik kötüleşmeye eşlik edebilir.

Faz ve pencere

Portal tanısal
Klinik olarak BT çekilmişse intravenöz kontrast sonrası portal venöz fazda bağırsak duvarı kalınlaşması ve komşu anslara göre azalmış ya da kaybolmuş kontrastlanma aranır. Pnömatozis, duvar çizgisi içinde lümendeki gazdan ayrı çizgisel veya küçük kistik odaklar şeklinde; portal venöz gaz ise mezenterik venlerden karaciğer içi portal dallara uzanan dallanmış gaz olarak görülür. Serbest intraperitoneal hava ve anslar arasındaki sıvı da bu fazda değerlendirilir; NEK için BT rutin ilk inceleme değildir.

BT bulguları

  • Bağırsak pnömatozisi — duvar içinde, lümen gazından ayrışan çizgisel veya küçük kistik gaz birikimleri; tek bir ansla sınırlı olabileceği gibi birden çok ansı tutabilir.
  • Portal venöz gaz — karaciğerin periferik dallarında dallanan gaz odakları şeklinde izlenebilir.
  • Bağırsak duvarında ödem — hastalıklı ans çevresindeki duvar kalınlaşır; komşu mezenterde ödem ve sıvı eşlik edebilir.
  • Duvar kontrastlanmasında azalma — komşu anslara göre belirgin hipokontrastlanan veya kontrastlanmayan segment ileri iskemi ve nekroz açısından kaygı vericidir.
  • Sabit veya genişlemiş ans — belirgin genişlemiş, içerikle dolu bir segment ve çevresindeki inflamatuvar değişiklikler ağır hastalıkta görülebilir; tek başına özgül değildir.
  • Pnömoperitoneum — bağırsak dışındaki serbest hava, NEK bağlamında perforasyon lehine doğrudan bulgudur.
  • Ultrasonografide kompleks serbest sıvı spontan intestinal perforasyonda görülebilir; tek başına özgül değildir ve bağırsak duvarı, perfüzyon, pnömatozis ile klinik bulgularla birlikte değerlendirilmelidir.
  • Modifiye Bell evrelemesinde pnömatozis ve portal venöz gaz dikkate alınsa da bu bulgular prematürelerde başka nedenlerle görülebilir; pnömoperitoneum evre IIIB NEK için tipik radyografik bulgudur. Modifiye Bell evrelemesinde klinik ve radyografik bulgular birlikte değerlendirilir; tek bir klinik, laboratuvar veya radyografik bulgu NEK tanısını tek başına kesinleştirmez.

Ölçütler ve sınıflamalar

Modifiye Bell evrelemesi
Modifiye Bell evrelemesi NEK şiddetini sınıflandırmak için kullanılan sistemlerden biridir; ancak tek bir klinik, laboratuvar veya radyografik bulgu tanıyı tek başına doğrulamaz. Alt evre tanımlarında farklılıklar bildirilmiştir; tek başına BT evre tayin etmez.

Normalde

Pnömatozis intestinalis bağırsak duvarı içindeki gazdır ve dışkıyla karışmış lüminal gazla karıştırılabilir. Şüpheli segmenti komşu anslarla duvar kalınlığı ve kontrastlanma bakımından yan yana kıyaslayın; karaciğer içinde gaz varsa portal dalların periferik uzanımını ayrıca kontrol edin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

segmental duvar kalınlaşması ve çevresel inflamasyon yapabilir; yenidoğandaki klinik bağlam ve duvar/portal gaz birlikteliği ayrımda önemlidir.

Tuzaklar

  • Dışkıyla karışmış lüminal gaz pnömatozis intestinalis sanılabilir; gazın bağırsak duvarı içinde yerleşip yerleşmediğini dikkatle değerlendirin.
  • Portal venöz hava periferik karaciğer dallarına ilerler; pnömobili daha santralde, hilusa yakın safra yollarında yoğunlaşır.
  • Umbilikal ven kateteri yerleştirilmesi veya manipülasyonu sonrasında portal sisteme hava girebilir; izole karaciğer gazını bağırsak bulgularından ayrı yorumlamayın.
  • Kontrastsız BT’de gazın yerini saptamak mümkün olsa da bağırsak duvarı perfüzyonu kıyaslanamaz; bu inceleme canlılık değerlendirmesini yanıtlamaz.
