Özet

Hematogen yayılımlı akut enfeksiyon çocukta sıklıkla uzun kemiğin metafizinde başlar; küçük çocukta damar bağlantıları nedeniyle epifiz ve komşu eklem de etkilenebilir. BT kemik iliği ödemini göstermez ve erken enfeksiyonda yararlı değildir; buna karşılık kortikal yıkım, periostal reaksiyon, sekestrum ve gazı gösterebilir. BT, kemik yıkımını göstermenin yanı sıra enfeksiyonla ilişkili yumuşak doku koleksiyonlarının değerlendirilmesine ve drenaj planlamasına yardımcı olabilir. Çocukluk çağı osteomiyelitinde tanı gecikmesi periostal apse ve büyüme plağı hasarı gibi komplikasyonlarla ilişkili olabilir; geç komplikasyonlar arasında kemik deformiteleri de bulunur.

Kemik algoritması tanısal
Kontrastsız ince kesit kemik rekonstrüksiyonunda metafizer litik odak, kortikal erozyon, periostal yeni kemik ve kronik olguda sklerotik sekestr ile onu çevreleyen involukrum seçilir; erken medüller hastalıkta BT normal olabilir.
Kontrastlı tanısal
IV kontrast sonrası yumuşak doku penceresinde koleksiyon düşük dansiteli merkez ve çevresel kontrastlanma gösterir; subperiostal yayılım ve kas kompartımanlarındaki abse sınırları değerlendirilir.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Metafizer medüllada coğrafi ya da düzensiz litik odak; başlangıçta korteks bütünlüğü korunabilir.
  • Endosteal oyulma ve kortikal erozyon, iltihabın medülladan kemiğin dış yüzüne ilerlediğini gösterir.
  • Subperiostal yeni kemik korteksi paralel izleyen ince tabaka veya daha kalın düzensiz kılıf biçiminde olabilir.
  • Sekestr, sklerotik çevre kemikten ayrılan yoğun nekrotik kemik fragmanıdır.
  • İnvolukrum, sekestr çevresinde gelişen kalın periostal yeni kemik kılıfıdır.
  • Kronik osteomiyelitte kloaka, sekestr ve involukrum gelişimi tanısal ipuçlarıdır.
  • BT, osteomiyelite eşlik eden yumuşak doku koleksiyonlarını gösterebilir.
  • İntramedüller ya da yumuşak doku gazı küçük hava odakları şeklinde görülür; travma veya girişim öyküsüyle birlikte yorumlanır.

Normalde

Normal uzun kemikte metafizer trabeküller düzenli, kortikal kabuk kesintisiz ve periost kemik yüzeyine yapışık ince çizgi olarak izlenir. Hastalıklı tarafı aynı seviyedeki karşı kemikle kıyaslarken medülladaki fokal radyolusensi, kortikal açıklığı ve periost altındaki yeni kemiği arayın; fizis hattını patolojik lizisle karıştırmayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
