Özet

Yabancı cisim yutulması özellikle küçük çocuklarda sık görülür; pil, kesici cisim ve birden fazla mıknatıs hızla ciddi doku hasarına yol açabilir. BT rutin inceleme değildir; perforasyon, fistül veya bağırsak tıkanıklığı kuşkusunda ya da ilk değerlendirmeden sonra tanı belirsiz kaldığında seçilmiş olgularda kullanılabilir. BT'de yabancı cisimlerin sayısı ve bağırsak anslarıyla ilişkisi ile arada sıkışmış olabilecek bağırsak duvarı birlikte değerlendirilmelidir. Birden fazla mıknatıs, bağırsak duvarlarında bası nekrozu, fistül veya perforasyona yol açabilir.

Faz ve pencere

Kontrastsız tanısal
Radyolüsen cisimleri ve lümen içindeki metalik/yüksek atenüasyonlu cisimleri doğrudan arayın; serbest hava, duvar çevresi yağ planı ve obstrüksiyon bulguları da değerlendirilebilir. İntravenöz kontrast, komplikasyon kuşkusunda duvar perfüzyonunu değerlendirmek için eklenebilir.
Portal
Komplikasyon şüphesinde bağırsak duvarı kontrastlanması, fokal duvar defekti, çevresel inflamasyon, koleksiyon ve damarlarla komşuluk değerlendirilir; rutin cisim saptama fazı değildir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Cismin tipi, şekli ve lümen içindeki konumu; özofagustan rektuma kadar kesitler boyunca izlenir.
  • Birbirine bitişik görünen metalik odakların sayısı ve farklı bağırsak ansları arasında uzanıp uzanmadığı belirlenir.
  • Mıknatıslar komşu bağırsak ansları arasında uzanıyorsa aradaki duvarın sıkışması ve perforasyon açısından değerlendirilmelidir.
  • Sivri cismin lümenden dışarı uzanan ucu, fokal duvar kesintisi ve komşu yağ dokusunda reaksiyon perforasyon kuşkusunu artırır.
  • Lümen dışı hava, lokal sıvı veya koleksiyon, özellikle cismin komşuluğunda ise komplikasyon lehinedir.
  • Proksimal ans dilatasyonu ve distal kollaps, cismin tıkayıcı konumda olduğunu düşündürür.
  • Düğme pil özellikle özofagusta nekroz, perforasyon ve fistül gibi ağır doku hasarlarına yol açabilir.

Normalde

Şüpheli kesitte cismi lümen gazı ve oral içerikten ayırın, sonra komşu ans duvarlarını, mezenter yağını ve peritoneal boşluğu aynı düzeyde karşılaştırın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
