Özet

Piriform sinüslerin değerlendirilmesi, farinksin karmaşık anatomisi ve fizyolojik hareketliliği nedeniyle güçleşebilir. Hipofarenks yakınmalarında görüntüleme, genellikle ilk aşamada yapılan görsel ve nazal endoskopik incelemeyi tamamlamak için kullanılır. Kalıcı fokal duvar dokusunu fizyolojik kollaps sanmak tanıyı geciktirebilir; yalnız lümen asimetrisini tümör saymak gereksiz ileri incelemeye yol açabilir.

Okuma sırası

  1. Aritenoidler ve piriform sinüsler. Hipofarenksin hyoid kemik düzeyinden krikoid kıkırdağın alt kenarı düzeyine uzandığını ve piriform sinüsün hipofarenks bölümlerinden biri olduğunu kesitsel görüntülerde tanımla.
  2. Dar piriform sinüs ve duvarı. Hipofarenksin üç bölümünü kesitsel görüntüde ayırt et: piriform sinüs, postkrikoid bölge ve posterior farengeal duvar.
  3. Kontralateral piriform sinüs. Piriform sinüsleri değerlendirirken farinksin karmaşık anatomisini ve fizyolojik hareketliliğini göz önünde bulundur; bunlar radyolojik değerlendirmeyi güçleştirebilir.

Kontrastsız
Farinksin fizyolojik hareketliliği, piriform sinüslerin radyolojik değerlendirmesini güçleştirebilir. Tek pozisyonlu kontrastsız inceleme duvar kontrastlanmasını değerlendirmez.

Önerilen pencereler: Yumuşak doku (G 400 / M 40).

BT bulguları

  • Piriform fossalar görüntülemede hava dolu çift yapılar olarak görülebilir; farinksin karmaşık anatomisi ve fizyolojik hareketliliği radyolojik değerlendirmeyi güçleştirebilir.
  • Kesitsel anatomi bilgisi, baş-boyun görüntülerindeki anatomik varyasyon ve patolojilerin yanlış yorumlanmasını önlemeye yardımcı olur.
  • Hipofarenks tümörlerinde BT, tümörün larengeal yumuşak dokulara ve kıkırdaklara penetrasyonunu doğrudan değerlendirebilir; görüntüleme ayrıca tümörün lenf nodlarına ve çevre dokulara uzanımını gösterebilir.

Normalde

Hipofarenks anatomisi karmaşıktır ve fonksiyonel hareketlere bağlı olarak şekil değiştirebilir; bu durum radyolojik değerlendirmeyi zorlaştırır. Piriform sinüs asimetrisini değerlendirirken baş-boyun kesitsel anatomisini göz önünde bulundur; anatomik varyasyonlar görüntülerin yanlış yorumlanmasına yol açabilir. Piriform fossalar görüntülemede hava dolu çift yapılar olarak görülebilir; farinksin karmaşık anatomisi ve fizyolojik hareketliliği radyolojik değerlendirmeyi güçleştirebilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
