Özet

Fitozis bulbi, ağır eski travma, inflamasyon veya başka bir göz hastalığı sonrasında gelişen küçülmüş ve işlevini yitirmiş glob görünümüdür. Orbita BT'sinde küçülmüş göz küresi ve intraoküler kalsifikasyonlar görülebilir. BT bulguları kronik hasarı gösterir; altta yatan spesifik etiyoloji genellikle klinik öykü ve diğer incelemelerle desteklenmelidir. Küçülmüş ve kalsifiye göz küresinin aktif intraoküler tümör veya açık kü yaralanması ile ayırt edilmesi gereklidir; yanlış tanı gereksiz enükleasyon veya gecikmiş acil tedaviye neden olabilir.

Faz ve pencere

Kontrastsız tanısal
Kontrastsız BT'de glob karşı tarafa göre küçük ve şekilsiz olabilir; duvar boyunca kavisli sklerokoroidal kalsifikasyonlar ve vitreus içinde kümelenmiş distrofik kalsifikasyonlar yüksek atenüasyonlu odaklar şeklinde görülür. Göz küresindeki sklerokoroidal kalsifikasyonlar ve hacim kaybı gibi kronik yapısal değişiklikler genellikle kontrastsız görüntüleme ile saptanır; kontrastlı incelemeler aktif inflamasyon veya tümör yayılımı değerlendirilmesinde değerlidir.
Kemik algoritması
Kemik penceresi sklerokoroidal kalsifikasyonları belirginleştirirken, yumuşak doku penceresi glob hacmi ve duvar yapısını değerlendirmeye yardımcı olur.

BT bulguları

  • Glob hacminde azalma — etkilenen göz küresi karşı taraftan küçük ve kısmen çökmüş olabilir.
  • Kronik hasar ve skarlaşma, göz küresinin normal oval konturunun bozulmasına ve şekilsizleşmesine yol açabilir.
  • Kavisli sklerokoroidal kalsifikasyon — arka veya çevresel glob duvarı boyunca ince, yoğun çizgisel odaklar.
  • Vitreus içi distrofik kalsifikasyon — glob merkezinde kümelenmiş veya kaba yoğun odaklar bulunabilir.
  • Yoğun glob içeriği — vitreus boşluğu homojen sıvı görünümünden daha yüksek atenüasyonlu ve düzensiz olabilir.
  • Lens deformasyonu veya yer değiştirmesi — eski travmatik ya da cerrahi hasar eşlik edebilir.

Normalde

Normal göz küresi, orbital duvarlar, yağ, kaslar ve damarlar arasındaki dengeyle stabil bir konumda ve düzenli oval konturda yer alır. Göz küresi boyutu, duvar konturu ve kalsifikasyonların dağılımı karşılaştırılarak hastalık açısından değerlendirilebilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Retinoblastom
