Nöroblastomun orbital metastazı

Özet

Nöroblastom orbital kemiklere metastaz yapabilir; orbital tutulum periorbital ekimoz veya proptozisle fark edilebilir. BT'de orbital kitleler, komşu kemiklerde erozyon, kalınlaşma, periost reaksiyonu veya litik defektler görülebilir; bu bulgular tek başına primer tümörü kanıtlamaz. Nöroblastomda orbital tutulumla birlikte kafatasında ve her iki orbitada çoklu kitleler bildirilmiştir. Orbital nöroblastom metastazlarında BT’de orbital kemiklerde kalınlaşma, periost reaksiyonu veya litik defektler ve eşlik eden yumuşak doku kitleleri görülebilir. BT'de orbital kitleler ve komşu kemik erozyonu görülebilir.

Kemik algoritması tanısal

BT kemik penceresinde orbital kitlelere komşu kemik erozyonu, kalınlaşma, periost reaksiyonu veya litik defektler değerlendirilir; görüntüye giren diğer kraniyal kemik odakları da gözden geçirilir.

BT bulguları

• Sfenoid duvar odağı — bir olguda sfenoid duvarlarda bilateral orbital kitleler bildirilmiştir.
• Spiküler periost reaksiyonu — kemik yüzeyinden dik uzanan ince kemik çıkıntıları güneş ışını benzeri görünüm oluşturabilir; bu görünüm tek başına nöroblastom için özgül değildir.

Normalde

Normal posterolateral orbital duvarda sfenoid kanat ve frontal kemik korteksi düzgün, kesintisizdir; kemik dış yüzünde spiküler çıkıntı ve komşu yumuşak doku kitlesi bulunmaz. Karşı tarafla aynı düzeyde kemik kalınlığını, periost konturunu, ekstrakonal yağ planını ve globun öne itilip itilmediğini kıyasla.

Orbital LCH

kalvaryumda soliter litik lezyon LCH için tipik bir görüntüleme bulgusudur; orbital LCH MRG'de belirgin kontrastlanma gösterebilir. Klinik bağlam, diğer odaklar ve MR bulguları ayrımda yardımcıdır.

Rabdomyosarkom

çocuklarda en sık görülen primer orbital malignitedir; hızlı büyüyen ağrısız bir kitle oluşturarak proptozise yol açar ve komşu kemiğe invazyon yapabilir.

Retinoblastom yayılımı

BT'de intraoküler kitlede kalsifikasyon görülebilir; MRG optik sinir boyunca yayılımı değerlendirmede daha duyarlıdır.

Lösemi infiltrasyonu

çocukluk çağı AML'sinde orbital kemiklerde spiküler güneş ışını benzeri periost reaksiyonu bildirilmiştir.

LCH, orbital tümörlerin ayırıcı tanısında yer alır; nöroblastomun orbital görüntüleme bulguları özgül olmadığından periost reaksiyonu tek başına tanıyı kesinleştirmez.

Tuzaklar

• Periorbital ekimozu yalnız travma olarak yorumlama; iki taraflı ekimoz, proptozis ve kemik lezyonları metastaz olasılığını gündeme getirir.
