Dalak psödokisti, çoğunlukla eski travma veya hematom sonrasında gelişen, epitel döşemesi olmayan kronik kistik lezyondur. Kontrastsız BT'de içeriğin sıvı ya da eski kanla ilişkili değişken atenüasyonu ve kalın, bazen çevresel kalsifiye duvarı gösterilebilir. Önceki travma öyküsü ve kalın/kalsifiye duvar gibi görüntüleme bulguları tanıyı destekler; kesin ayırım için histopatolojik değerlendirmeye ihtiyaç duyulur. Kistin büyümesi veya komplikasyon gelişiminde enfeksiyon, rüptür ve kitle etkisi yönetimi yönlendirir; hemorajik değişimler de gözden kaçırılmamalıdır.

BT bulguları
- Dalak parankiminde iyi sınırlı, yuvarlak veya oval kistik boşluk
- Uniloküler ya da multiloküler, çoğunlukla sıvı atenüasyonlu içerik
- Kalın, düzensiz veya tam olmayan periferik duvar kalsifikasyonu
- Eski kanama/protein içeriğiyle uyumlu heterojen ya da basit sıvıdan yüksek atenüasyon
- Önceki dalak travması veya enfeksiyonu sık görülen nedenler arasındadır.
- Çevre yağ dokusunda inflamasyon ya da sıvı yeni komplikasyonu düşündürür
- Büyük lezyonda komşu mide, böbrek ve diyaframda yer değiştirme
Normalde
Normal dalak homojen parankim yapısı gösterir ve içinde kalın kalsifiye bir kist duvarı ya da kavite bulunmaz. Kistik boşluğun sınırını dalak kapsülünden ayır; özellikle periferik kalsifikasyonun kaviteyi çevreleyip çevrelemediğini ve komşu parankimde travma sekeli olup olmadığını incele.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar
- Kalsifiye duvar tek başına psödokist tanısı koydurmaz; hidatik ve epidermoid kistlerde de duvar kalsifikasyonu olabilir.
- Basit içerikli travma sonrası kist, görüntüleme bakımından epidermoid kistten ayrılamayabilir; travma öyküsü ve eski görüntüler tanısal bağlam sağlar.
- Görüntüleme özellikleri bazı kist türleri arasında örtüşebildiğinden, kesin ayırım genellikle histopatoloji gerektirir.
