Özet
Jejunoileal divertikülit, çoğunlukla mezenterik kenardaki edinilmiş bir divertikülün inflamasyonudur ve akut karın ağrısını taklit edebilir. Kontrastlı BT'de tanı için asıl hedef, çevre yağdaki değişikliklerin merkezinde yer alan hastalıklı divertikülü ve onun hangi ansa açıldığını göstermektir. Apse, sınırlı perforasyon veya serbest perforasyon gelişirse görüntüleme aciliyetini ve cerrahi değerlendirme gereksinimini etkiler. Yalnız fokal barsak duvar kalınlaşması olarak raporlamak apandisit, Crohn hastalığı ya da neoplaziyle karışmaya ve gerçek odağın atlanmasına yol açabilir.
Faz ve pencere
- Portal tanısal
- İntravenöz kontrast sonrası kalın/hiperemik divertikül duvarı, kese içindeki hava-sıvı ya da fekalize içerik, komşu mezenterik yağda kirlenme ve varsa rim kontrastlanan apse aranır; ince barsak duvarının kontrastlanması ve ekstralüminal gaz da incelenir.
Önerilen pencereler: Batın (G 400 / M 50).
BT bulguları
- Mezenterik kenardan çıkan, komşu jejunal veya ileal lümene boyunla bağlanan fokal kese; inflamasyonun merkezini bu kese oluşturur.
- Divertikül duvarında kalınlaşma ve hiper-kontrastlanma; lümeninde hava, sıvı, kontrast veya fekalize içerik bulunabilir.
- Divertikül çevresindeki mezenterik yağda belirgin çizgilenme ve yoğunluk artışı; inflamasyon odağına yakın fasya da kalınlaşabilir.
- Komşu ince barsak ansında eksantrik duvar kalınlaşması, ağır çevresel inflamasyonda görülebilir.
- Halka biçiminde kontrastlanan, sıvı ve gaz içeren koleksiyon apseyi; koleksiyon dışındaki sınırlı ekstralüminal gaz odağı sınırlı perforasyonu düşündürür.
- Yaygın serbest intraperitoneal hava veya sıvı, periton irritasyonu ve perforasyon açısından kritik eşlikçi bulgudur.
- İnce barsak duvarında incelme, arteriyel iskemiyle ilişkili olabilir.
Normalde
Normal jejunal veya ileal ansın mezenterik kenarında lümenle ilişkili kese bulunmaz; mezenterik yağ homojen ve komşu fasya incedir. Şüpheli odağın merkezinden ans lümenine doğru boynu izleyin; duvar kalınlaşması ve yağlı doku reaksiyonunun, aynı ansın karşı duvarına ve yakın normal anslara göre fokal olup olmadığını karşılaştırın.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

- Epiploik apandajit
- kolona komşu oval yağ dansiteli odak ve ince hiperdens halka görülür; ince barsak lümeniyle bağlantısı olmaz.
Tuzaklar
- Komşu ans distansiyonu divertikül boynunu saklayabilir; aksiyel kesitlerde seçilemeyen kese koronal ve sagittal planda belirginleşebilir.
- Mezenterik kenardaki inflamatuvar kitle apse veya tümör gibi görünebilir; lümene bağlanan divertiküler yapı ve merkezdeki gaz/fekal içerik odağı tanıyı netleştirir.
- İnce bağırsak divertikülü çevresindeki ekstralüminal gaz sınırlı perforasyonda yerel kalabilir; gazın kese çevresinde sınırlı mı, peritonda yaygın mı olduğunu değerlendirin.
