Özet

Perianal fistül ve apse, anal bez kökenli enfeksiyonun çevre yağ planlarına ve sfinkter kompartımanlarına uzanan bulgularıdır; Crohn hastalığında çoklu ve dallanan traktlar görülebilir. Acil BT, özellikle derin koleksiyonun yerini, gazı ve pelvis ya da ischioanal fossaya yayılımı hızla gösterir. İntravenöz kontrastlı BT’de rim kontrastlanması koleksiyonun seçilmesini kolaylaştırır, ancak küçük fistül traktlarının haritalanmasında MR daha duyarlıdır. Supralevator uzanımın ya da ek bir apse cebinin atlanması hastalığın gerçek yaygınlığını ve aciliyetini olduğundan az gösterebilir.

Portal tanısal
Kontrastlı pelvik BT’de perianal, intersfinkterik veya ischioanal yağ içinde düşük atenüasyonlu sıvı merkezinin çevresinde kontrastlanan duvar; aktif fistül traktında anal kanaldan cilde uzanan kontrastlanan lineer yumuşak doku seçilebilir. Kontrastlı BT, apseleri ve derin pelvik yayılımı değerlendirmeye yardımcı olur.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Batın (G 400 / M 50).

BT bulguları

  • Perianal veya intersfinkterik kompartımanda merkezî sıvı atenüasyonu ve çevresel kontrastlanan apse duvarı
  • İç anal açıklık ile perianal cilt arasında ilerleyen kontrastlanan lineer trakt
  • Traktın internal ve eksternal sfinkterlere, intersfinkterik düzleme ve ischioanal fossaya göre seyri
  • Levator ani altı ile üstü arasındaki ilişki ve supralevator pelvik uzanım
  • Birbirine bağlanan yan dallar, at nalı biçimli yayılım veya birden fazla koleksiyon

Parks fistül sınıflaması
Parks sınıflaması primer fistül traktlarını intersfinkterik, transsfinkterik, suprasfinkterik ve ekstrasfinkterik olmak üzere dört örüntüde tanımlar.
St James's University Hospital MR derecelendirmesi
St James's MR derecelendirmesinde derece 1 basit lineer intersfinkterik fistül; derece 2 intersfinkterik apse veya ikincil trakt, derece 3 transsfinkterik fistül, derece 4 transsfinkterik fistüle ischioanal fossada apse veya ikincil trakt eşlik etmesi, derece 5 ise supralevator ve translevator hastalık olarak tanımlanır.

Normalde

Normal anal kanal çevresinde internal ve eksternal sfinkter halkaları simetrik yumuşak doku yoğunluğunda izlenir; intersfinkterik düzlem, ischioanal yağ ve levator ani altındaki perianal yağ temizdir. Aynı aksiyel seviyede anal lümen çevresindeki halkaları takip et; yağ içinde rim kontrastlanan sıvı odağı, çizgisel trakt veya tek taraflı yağ silinmesi normal görünümden ayrılır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Hidradenitis suppurativa

Tuzaklar

  • BT’de küçük, ince fistül traktı görünmeyebilir; belirgin apse yokluğu fistülü dışlamaz ve klinik kuşkuda MR haritalaması gerekebilir.
  • Anal kanal duvarındaki tek bir damar veya parsiyel hacim görüntüsü trakt sanılabilir; çizginin ardışık kesitlerde izlenmesini ve cilt/kanal bağlantısını doğrula.
  • Levator ani düzeyini aksiyel tek kesitte kaçırmak supralevator yayılımı gizler; koleksiyonun kraniokaudal uzanımını koronal ve sagittal rekonstrüksiyonlarda takip et.
  • Drenaj sonrası hava, cerrahi materyal ve açık yara traktı enfekte kaviteye benzeyebilir; işlem öyküsü ve cilt kesisiyle mekânsal ilişkiyi dikkate al.
