Özet

Bouveret sendromu, çoğunlukla kolesistoduodenal fistülden geçen safra taşının pilor veya proksimal duodenumu tıkamasıyla gelişen nadir bir safra taşı ileusu biçimidir. BT, ektopik taşların sayı, boyut ve yerleşimini ve obstrüksiyonun kesin seviyesini göstererek tanıyı ve ameliyat öncesi planlamayı destekler. Taş izodens olabilir; bu nedenle yalnız taşın görünmesine güvenmek, mide çıkışındaki geçiş noktasını ve safra yollarındaki havayı gözden kaçırabilir. Sendrom, özellikle ileri yaş ve komorbiditesi bulunan hastalarda yüksek morbidite ve mortaliteyle ilişkilidir; bildirilen mortalite oranları kaynaklarda %12–30 arasında değişmektedir.

Kontrastsız tanısal
Ektopik taş BT'de her zaman belirgin olmayabilir; taş boyutunu değerlendirirken duodenum lümeninin bağımlı kesimindeki sıkışmış hava, kalsifiye kenarı çevreleyen yumuşak doku ve taş içindeki silik radyolüsent katmanlar ile safra ağacındaki hava birlikte incelenebilir. İzodens taş kontrastsız kesitte de silik kalabilir.
Portal tanısal
Kontrastlı BT'de proksimal duodenumda impakte ektopik taş ve buna bağlı mide çıkış obstrüksiyonu ile çevre inflamatuvar değişiklikler görülebilir.

BT bulguları

  • Ektopik kalkül pilor veya proksimal duodenum lümeninde, özellikle bulbus düzeyinde impakte olabilir; BT'de taşın çevresinde kalsifiye kenar bulunabilir.
  • Taşın hemen proksimalinde belirgin sıvı ve gazla genişlemiş mide; distal ince bağırsak ansları görece sönük kalır
  • Mide çıkışında ani kalibrasyon değişikliği ve taşla aynı seviyede mekanik geçiş noktası
  • Sol intrahepatik kanallarda, ana safra kanalında veya safra kesesinde lümene uyan dallanan gaz odakları (pnömobili)
  • Küçülmüş/kontrakte safra kesesi, keseyle duodenum arasında silinmiş yağ planı veya görülebilir fistül ağzı
  • Komşu duodenum duvarında ödem ve çevre yağ dokuda inflamatuvar dansite artışı
  • Mide çıkış obstrüksiyonu, pnömobili ve ektopik taş birlikteliği (Rigler triadı) tanısal temel bulgulardır.
  • Proksimal duodenum çevresinde inflamatuvar değişiklikler eşlik edebilir.

Normalde

Normal mide antrumu pilora kesintisiz açılır; duodenum bulbusu ve inen kısmın lümeni içerik miktarına göre değişse de taş biçimli sabit dolum defekti ve proksimal retansiyon oluşturmaz. Kıyaslarken mide distansiyonunu, pilor-duodenum lümenindeki kalkülü, safra kesesi hacmini ve safra yollarındaki havayı birlikte ara.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Kolesterol ağırlıklı taş safra ile benzer atenüansta kalabilir; yalnız hiperdens kalkül aramak yerine lümen konturunu, geçiş noktasını ve komşu kesitlerdeki taş etkisini izle.
  • Safra yollarındaki hava iatrojenik girişimler veya prior anastomozlar nedeniyle de görülebilir; tanı için taş ve fistül bulgularının birlikteliği gereklidir.
  • Taş boyutunun ve özelliklerinin doğru değerlendirilmesi için duodenal lümenin bağımlı kısmındaki hava, kalsifiye kenarı çevreleyen yumuşak doku alanı ve taş içindeki zayıf radyolüsent katmanlara dikkat edilmelidir.
  • Kolesistoduodenal fistül BT'de doğrudan izlenmeyebilir; bir olguda fistül traktı gösterilememiş, safra yollarındaki hava ve safra kesesi-duodenum komşuluğundaki inflamasyon eşlik etmiştir.
