Özet

Sternumda görülen her çizgi, basamak veya kemik açıklığı travmatik kırık değildir; manubriosternal eklem, gelişimsel ossifikasyon merkezleri, sternal foramen ve cleft benzer görünüm oluşturabilir. BT, çoklu planar ve üç boyutlu reformasyonlarıyla sternal korteks bütünlüğünü ve eklem hizasını; yumuşak doku penceresi ise komşu retrosternal değişiklikleri değerlendirmek için kullanılır. Özellikle çocuklarda birleşmemiş ossifikasyon odakları, erişkinde eklem yüzü ve orta hat foramen yanlış pozitif kırık yorumuna yol açabilir. Sternal varyantların patolojiyle ayırımı gereksiz tanı ve tedaviyi önlerken; gerçek travmalarda eşlik eden ciddi toraks yaralanmalarının birlikte değerlendirilmesi gerekir.

BT bulguları

  • Akut sternal kırık yer değiştirmiş olabilir; manubriosternal dislokasyon eşlik edebilir.
  • Retrosternal hematom, özellikle kırıkla aynı seviyede anterior mediastinal yumuşak doku dansite artışı olarak izlenir ve travmatik yaralanma lehine bağlamsal bulgudur.
  • Çocuklarda sternal ossifikasyon merkezleri birleşmemiş olabilir; sternumun birleşmemiş lateral ossifikasyon merkezleri, honeycomb sternum adı verilen normal anatomik varyantı oluşturur.
  • Sternebra ossifikasyon merkezleri segment içinde düzenli konumlanır ve çevreleri düzgün/simetrik olabilir; birleşmemiş merkezler gelişim çağında fraktür fragmanını taklit eder.
  • Sternal foramen gibi sternal varyasyonların tanınması, bunları patolojik durumlardan ayırmak açısından önemlidir.
  • Sternal cleft, sternal barların embriyogenez sırasında birleşmemesine bağlı konjenital bir göğüs duvarı defektidir.

Normalde

Sternumda farklı görünüm ve sıklıkta anatomik varyasyonlar görülebilir; bunların patolojik durumlardan ayırt edilmesi önemlidir. Sternal kırık, ağır travmanın dolaylı bir bulgusu olabilir ve vasküler yapıların dikkatle değerlendirilmesini gerektirir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
