Özet

Astım veya kistik fibrozis zemininde gelişen alerjik bronkopulmoner aspergilloziste (ABPA), koyu ve yoğun mukus bronşları tıkayabilir. İnce kesit BT, mukus tıkaçlarını ve bronşektazinin tipini ve dağılımını değerlendirmede kullanılır; kontrastsız BT'de mukusun paraspinal kastan daha yoğun olup olmadığı belirlenebilir. Tıkaç paraspinal kastan daha yoğun görünüyorsa bu, ABPA lehine çok özgül bir bulgudur; tek başına klinik ve immünolojik tanı sürecinin yerini tutmaz. Tıkanmanın gözden kaçması distal atelektaziye, tekrarlayan enfeksiyona veya lob kollapsına yol açabilir.

Kontrastsız tanısal
Kontrastsız toraks BT'de paraspinal kastan daha yoğun mukus, yüksek atenüasyonlu mukus (HAM) olarak tanımlanır; ince kesit BT mukus tıkaçlarını ve bronşektazinin tipini ve dağılımını değerlendirmede kullanılır. Mukus atenüasyonu için intravenöz kontrast gerekmez.
HRCT tanısal
İnce kesit BT, mukus tıkacıyla dolu santral bronşları ve mukusla dolu bronşektazilerin parmak-eldiven görünümünü gösterebilir; HRCT'de sentrilobüler nodüller de görülebilir.

BT bulguları

  • Paraspinal kastan yoğun mukus — bronş lümenindeki içerik kontrastsız mediasten penceresinde komşu paraspinal kastan daha beyaz görünür; ABPA için tanımlayıcı bulgudur.
  • Dallanmış mukus tıkacı — bronş ağacının şeklini izleyen tübüler opasite, kesitler boyunca dallanan görünüm kazanabilir.
  • Parmak-eldiven görünümü — genişlemiş bronşların mukusla dolması, hilustan perifere uzanan dallı opasiteler oluşturur.
  • ABPA'da santral bronşektazi tipik olarak segmental veya subsegmental bronşları tutar ve üst loblarda baskın olabilir; mukusla dolu bronşektaziler parmak-eldiven görünümü oluşturabilir.
  • Mukusla tıkalı bronş distalinde sentrilobüler nodüller görülebilir.
  • Distal hacim kaybı — tam tıkanmış bronşun beslediği segmentte artmış atenüasyon, bronş damar demeti yakınlaşması ve fissür çekilmesi gelişebilir.

Normalde

Yüksek atenüasyonlu mukusu değerlendirmek için kontrastsız BT'de bronş içeriğinin atenüasyonunu paraspinal kasla karşılaştırın; kastan daha yoğun mukus HAM olarak tanımlanır. Aynı düzeyde paraspinal kası referans alarak bronş içeriğinin atenüasyonunu kıyaslayın; normalde bronş lümeninde kastan yoğun bir materyal bulunmaz.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Kalsifiye bronş taşı
lümen içinde çok yoğun, keskin sınırlı kalsifik odak yapar; çevresinde mukus gibi dallanan yumuşak doku kolonu beklenmez.
Bronş içi aspirasyon materyali
çoğu kez bağımlı bronşta görülür ve klinik aspirasyon öyküsüyle anlam kazanır; paraspinal kastan yoğun mukus örüntüsü ABPA'yı daha çok destekler.
Endobronşiyal tümör
trakeobronşiyal lezyonlar BT'de bronş duvarı kalınlaşması veya endolüminal nodül şeklinde görülebilir; karsinoid tümörlerde kalsifikasyon bulunabilir. ABPA/ABPM şüphesinde tanı belirsizse bronkoskopi düşünülebilir.
Pulmoner aspergilloma
tüberküloz veya KOAH bulunan hastalarda akciğerde nodüler kitle şeklinde görülebilir.
Yüksek atenüasyonlu parankimal birikim
kalsifiye nodül ya da ossifikasyon bronş lümeni dışında yer alır; multiplanar kesitlerde hava yolu ile süreklilik göstermez.

Tuzaklar

  • Hafif kalsifikasyonu değerlendirmede mediasten penceresi kullanılabilir; yüksek atenüasyonlu mukus, kontrastsız BT'de paraspinal kastan daha yoğun olmasıyla tanımlanır.
  • Yüksek atenüasyonlu mukus ABPA için çok özgül olsa da yokluğu ABPA'yı dışlamaz; olağan atenüasyonlu tıkaçlarda da klinik bağlam önemlidir.
  • Koronal rekonstrüksiyon, akciğer hastalıklarında dağılımı değerlendirmeye yardımcı olabilir.
  • ABPA/M şüphesinde tanı belirsizse bronkoskopi düşünülebilir.
