Özet
Kronik berilyum hastalığı, uzun süreli mesleki berilyum maruziyetine karşı gelişen ve başlıca akciğerleri tutan granülomatöz bir hastalıktır. BT, akciğer parankimi ve mediastendeki bulguların yerini ve yaygınlığını değerlendirmeye yardımcı olur; radyolojik görünüm sarkoidozla benzer olabilir. Öykü ve BT bulguları olası tanıları göstererek ileri incelemeleri yönlendirebilir; BT tek başına berilyum duyarlanmasını kanıtlamaz. Maruziyet öyküsünün sorgulanmaması tanının sarkoidoz olarak kalmasına ve mesleki hastalığın gözden kaçmasına neden olabilir.

BT bulguları
- Perilenfatik mikronodüller — fissür, interlobüler septa ve bronkovasküler demetler boyunca dizilir; rastgele dağılımdan ayrılır.
- Üst ve orta zon baskınlığı — perihiler ve üst lob parankimi daha yoğun tutulabilir.
- Bilateral hiler ve mediastinal lenfadenopati — sarkoidozla ortak bir bulgudur ve berilyum hastalığına özgü değildir.
- Bronkovasküler/septal nodüler kalınlaşma — lenfatik yolları izleyen nodüller peribronkovasküler ve fissüral yüzeylere yerleşir.
- Kronik berilyum hastalığında nodüller bronşlar çevresinde kümelenebilir.
- Kronik berilyum hastalığında BT'de çoklu nodüller görülebilir; bunlar bronşlar, interlobüler septalar veya subplevral bölgede kümelenebilir.

Normalde
Normal akciğerde fissürler ince çizgi, interlobüler septalar ise çoğunlukla seçilemeyen yapılar olarak izlenir; bronkovasküler demetlerin çevresi nodülsüzdür. Üst lob kesitlerinde fissür, plevra ve bronş-damar demetleri boyunca nodül olup olmadığını sistematik kıyaslayın; lenf düğümlerini komşu mediastinal yağ planlarından ayrı sınırlandırın.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

- Lenfoid interstisyel pnömonide dallanmayan sentrilobüler nodüller görülebilir; bu dağılım, fissür, septa ve plevra boyunca uzanan perilenfatik paternden farklıdır.
- Miliyer enfeksiyon veya metastaz
- nodüller lenfatik yapılara bağlı değil, akciğer boyunca rastgele dağılır.
Tuzaklar
- BT görünümü sarkoidozla örtüşebilir; berilyum hastalığını yalnız görüntüden kesinleştirmeyin ve mesleki maruziyeti özellikle sorgulatın.
- Sentrilobüler nodülleri perilenfatik dağılım sanmayın; gerçek perilenfatik nodüller fissür, septa ve plevra yüzeyinde de izlenir.
- Hiler lenfadenopati tek başına özgül değildir; nodül dağılımını ve akciğer dışı eşlik eden bulguları birlikte okuyun.
- Üst lob fibrozisi ilerleyici olabilir; kronik distorsiyonu aktif enfeksiyon veya yeni malign odak olarak yanlış yorumlamamak için önceki BT ile karşılaştırın.
- Berilyum hastalığı geleneksel ve geleneksel olmayan mesleki ortamlarda görülebildiğinden, öyküde mevcut ve geçmiş işlerdeki olası berilyum maruziyeti sorgulanmalıdır.
