Özet

Yassı hücreli karsinom özofagusun herhangi bir düzeyinde gelişebilir ve torasik hastalıkta mediastinal organlara yakınlığı lokal yayılımın değerlendirilmesini önemli kılar. Kontrastlı BT, primer kitlenin uzunluğunu, bölgesel lenf düğümlerini ve uzak hastalığı haritalarken trakeobronşiyal ağaç, aorta ve perikardla ilişkisini gösterir. Başlangıç evrelemesi klinik bağlama göre FDG PET/BT ile tamamlanabilir; PET/BT, BT’de görülmeyen uzak hastalığın araştırılmasına katkı sağlar, ancak duvar katmanlarını veya fistül kanalının anatomisini göstermenin yerini tutmaz. BT duvar katmanlarını ayıramadığı için erken T evresini tek başına belirleyemez; endoskopi, biyopsi ve gerektiğinde endoskopik ultrasonla tamamlanır. Hava yoluna invazyon veya özofagotrakeal fistülün gözden kaçması aspirasyon ve sepsis riskini, uzak yayılımın atlanması ise evrelemeyi yanlış yönlendirebilir.

Faz ve pencere

Venöz tanısal
İntravenöz kontrastlı toraks ve üst abdomen incelemesinde düzensiz/asimetrik mural yumuşak doku, lümen daralması, komşu mediastinal yağ planları, kontrastlanan lenf düğümleri ve karaciğer-akciğer metastazları değerlendirilir.

Önerilen pencereler: Mediasten (G 350 / M 50), Akciğer (G 1500 / M -600), Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Özofagus duvarında fokal, eksantrik veya çevresel yumuşak doku kalınlaşması ve düzensiz dış kontur.
  • Tümör segmentinde asimetrik lümen daralması, keskin kenarlı ve düzensiz mukoza konturu.
  • Tümör çevresindeki mediastinal yağın silinmesi ve komşu organ konturunda çekinti ya da bası; tek başına temas invazyon kanıtı değildir.
  • Trakea veya ana bronş duvarında kalınlaşma, lümene uzanan yumuşak doku ya da özofagusla hava yolu arasında doğrudan fistül traktı.
  • Özofagus çevresi, paratrakeal, subkarinal veya üst abdomen lenf düğümlerinde büyüme ya da nekrotik görünüm; boyut tek başına metastazı doğrulamaz.
  • İleri hastalıkta hematogen yayılım sık görülür; karaciğer ve akciğer en sık uzak metastaz bölgeleridir.
  • Komşu mediastinal organ invazyonunu değerlendirirken, özellikle hava yolu için fistül veya lümen içi tümör uzanımını, duvar kalınlaşmasını ve komşu hava yolunda bası ya da yer değiştirmeyi arayın; tümörün komşu yapıya dayanması veya yağ planı kaybı tek başına invazyonu kesinleştirmez.

Ölçütler ve sınıflamalar

Özofagus duvar kalınlığı: lümen distansiyonuna bağlı üst normal sınır
Kontrastlı BT'de özofagus duvarının iç katmanları güvenilir şekilde ayrışmaz; bu nedenle duvar kalınlığı için sabit milimetre eşiği yerine tümörün periesofageal yağa invazyonu ve komşu yapılarla ilişkisi esas alınır.

Normalde

Kontrastlı BT'de normal özofagus duvarı katmanları ayırt edilmez; sağlıklı durumda periesofageal yağ planları korunurken, malignite durumunda tümör-duvar kalınlaşması ve komşu organlardaki yağ aralığının kaybı öne çıkar. Karşılaştırmada tümör çevresindeki dış kontur, mediastinal yağ aralığı ve trakea, ana bronş, aorta ile perikardın ayrı sınırlarının korunup korunmadığına bakılır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

çoğunlukla distal özofagus ve gastroözofageal bileşke merkezlidir.
Enfeksiyöz özofajit genellikle immünosupresif hastalarda görülür ve en sık etkenleri HSV, CMV ve Candida türleridir; BT'deki spesifik morfolojik bulgular kaynaklarda detaylandırılmaz.
Özofagus lenfoması
yaygın çevresel kalınlaşma ve başka istasyonlarda lenfadenopati eşlik edebilir.
Submukozal GIST/leiomyom
lümeni iten, düzgün sınırlı kitle ve görece korunmuş mukoza.
Akalazya
distal düzgün daralma ile birlikte kitle oluşturmayan yaygın özofagus genişlemesi.

Tuzaklar

  • Mukoza yüzeyi korunmuş superficial lezyonlar BT'de belirgin kitle oluşturmaz; erken dönem hastalıkta çok hafif bulgular görülebilir ve şüpheli durumlarda endoskopik değerlendirme tamamlamayı gerektirir.
  • Tümörün aorta veya hava yoluna değmesi doğrudan invazyon demek değildir; yağ planı, organ konturundaki değişiklik ve lümen içi uzanımı birlikte ara.
  • Normal boyuttaki lenf düğümleri mikrometastaz taşıyabilir; büyük düğümler de reaktif olabilir, bu nedenle BT boyutunu patolojik kesinlik olarak sunma.
  • Trakeoözofageal fistüller lokal ileri hastalığın bilinen komplikasyonudur ve tanı genellikle bilgisayarlı tomografi ile endoskopik/bronşkopik doğrudan görüntülemenin kombinasyonu ile konur.
