Özet

Pelvik venöz bozukluklar over veya internal iliak venlerde genişleme ve işlev bozukluğuyla ilişkili olabilir; pelvik ven genişlemeleri asemptomatik de görülebildiğinden görüntüleme bulguları hastanın yakınmalarıyla ilişkilendirilerek yorumlanır. Kontrastlı BT, uterus ve çevresindeki genişlemiş, kıvrımlı venöz yapıları ve renal ya da iliak ven düzeyindeki olası darlıkları göstermeye yardımcı olur; venöz akımın dinamik değerlendirilmesinde BT sınırlıdır, Doppler US ve venografi reflüyü değerlendirebilir. BT bulguları klinik yakınmalarla uyumlu bir venöz anatomi örüntüsünü tanımlamaya yarar, tek başına sendrom tanısı koymaz. Belirgin venöz kollaterallerin altında yatan renal veya iliak ven obstrüksiyonu atlanırsa konjesyonun nedeni açıklanamayabilir.

Faz ve pencere

Portal tanısal
Kontrastlı venöz opasifikasyonda parauterin pleksusta serpijinöz, değişken kalibrasyonlu damarlar ve genişlemiş over venleri izlenir; damarların devamlılığı renal ven, iliak ven ve vena kava çıkışlarına kadar izlenerek olası kompresyon ya da kollateral yol araştırılır. Tek fazlı BT venöz akımın dinamik değerlendirmesini sağlamaz; reflünün gösterilmesi ve derecelendirilmesinde Doppler US, zaman çözünürlüklü MR anjiyografi veya venografi daha elverişlidir. Tek fazlı BT reflü yönünü doğrudan göstermez.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • BT'de uterus, adneksler ve geniş bağ çevresinde kıvrımlı, genişlemiş ve kontrastlanan tübüler venöz yapılar görülebilir.
  • Over veninin olağandan geniş kalibrasyonu; çap ölçümü damar eksenine dik, kontrastla dolu lümenden yapılır.
  • Pelvik yan duvar ve adneks çevresinde serpijinöz venöz kollaterallerin birden çok kesitte sürmesi.
  • Uterus arkasında ve vajen çevresinde pleksusların kümelenmesi; dağılımı tek kistik adneks kitlesinden ayır.
  • Sol renal venin aortomezenterik aralıkta daralması veya iliak venlerin kompresyonu gibi olası çıkış engelleri.
  • Over veni boyunca trombüs veya kontrastlı BT'de lüminal dolum defekti görülmesi, over ven trombozu olasılığını düşündürür ve ayrıca değerlendirilmelidir.
  • Pelvik varisler belirgin olsa da BT'de görünür genişleme kronik pelvik ağrı sendromunu tek başına doğrulamaz.

Normalde

Pelvik venöz drenaj kişiden kişiye değişebilir; uterusun alt bölümü başlıca internal iliak venlere, üst bölümü ise uterin veya over pleksusu üzerinden over venlerine drene olur. Pelvik venöz genişlemeyi değerlendirirken ipsilateral parauterin venlerin sayısı ve çapı ile over veni çapını kaydet; önerilmiş BT ölçütleri en az dört ipsilateral pelvik venden en az birinin 4 mm'den büyük olmasını veya over veninin 8 mm'den büyük olmasını içerir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Uterin miyomlar
miyometriuma bağlı solid kitle ve kontur deformasyonu görülür; parametriumdaki damar kanallarıyla karıştırılmamalıdır.
Pelvik arteriyovenöz malformasyonda şantın venöz çıkış damarı ve akımı anjiyografide değerlendirilebilir; verilen kaynaklar BT'de erken arteriyel doluş için tanısal ölçüt sunmuyor.
İliak ya da renal ven obstrüksiyonuna bağlı kollateraller
venöz ağ benzer görünebilir; merkezi darlık veya bası seviyesini ve kollateral yönlenmesini araştır.
