Özet

Ayak yumuşak dokularındaki gaz, ülser veya penetran yaralanmadan dış ortamla girişe bağlı olabileceği gibi gaz oluşturan enfeksiyon ve nekrotizan yumuşak doku enfeksiyonunda da görülebilir. BT, gazı küçük cepler halinde saptar ve cilt altı, derin fasya, kas, tendon kılıfı veya kemik içindeki dağılımını haritalar. Gazın derin fasyada yayılması, fasya kalınlaşması, sıvı ve kontrastlanmayan doku ile birlikteyse ciddi enfeksiyon lehinedir; gaz yokluğu erken nekrotizan enfeksiyonu dışlamaz. İntraosseöz gaz, gaz oluşturan kemik enfeksiyonu olan amfizematöz osteomiyeliti düşündürür; bu nadir ve agresif enfeksiyonda erken tanı önemlidir.

Faz ve pencere

Kontrastsız tanısal
Kontrastsız kesitlerde gaz, yumuşak doku içinde çevre dokudan belirgin düşük atenüasyonlu siyah odaklar veya çizgisel cepler olarak görülür; cilt altından derin fasya, kas ve kemik medullasına uzanımı ve ülser/penetrasyonla bağlantısı izlenir.
Kontrastlı tanısal
Kontrastlı BT’de selülitte cilt altı ödemi ve yağ dokusunda çizgilenmeye ek olarak diffüz kontrastlanma veya kontrastlanan retiküler septasyonlar görülebilir; bunlar bland ödemden ayrımına yardımcı olur. Apse, çevresel kontrastlanan duvarı bulunan merkezi hipodens koleksiyon olarak izlenebilir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).

BT bulguları

  • Fasya boyunca gaz — küçük kabarcık veya çizgisel hava, yüzeyel yağ dokusunun ötesinde derin fasya kompartımanlarını izler.
  • Selülitte cilt kalınlaşması, cilt altı ödemi ve yağ dokusunda retiküler çizgilenme görülebilir; yüzeyel fasya da ödemli ve kalınlaşmış olabilir.
  • Derin fasya ve kas tutulumu ile cilt altı gazı nekrotizan yumuşak doku enfeksiyonu olasılığını artırır; BT tek başına kesin erken tanı sağlamadığından bulgular klinik değerlendirmeyle birlikte yorumlanmalıdır.
  • Kas planı tutulumu — intramüsküler ödem, gaz, heterojen kontrastlanma veya nekrotik kontrastlanmayan alan olabilir.
  • Ülser veya penetran giriş — cilt defekti, yabancı cisim ya da travma kanalından gazın dokuya girdiği yol izlenebilir.
  • İntraosseöz gaz — kemik medullasında hava odakları, açık cilt yarasıyla açıklanmıyorsa amfizematöz osteomiyelit açısından önemlidir.
  • Gaz görülmemesi nekrotizan fasiiti dışlamaz; BT’de yumuşak doku gazı bulunmayan olgular da olabilir.
  • Nekrotizan fasiitte kontrastlı MRG'de kontrastlanmayan nekrotik dokular görülebilir; BT'de yumuşak doku gazı görülmemesi NF'yi dışlamaz.

Normalde

Normal ayak yumuşak dokusunda cilt altı yağ, plantar fasya, tendon kılıfları ve kas planları arasında hava bulunmaz; kas ve kemik medullasında gaz odağı beklenmez. Karşı taraf aynı düzeyde fasya kalınlığını ve yağ planlarını kıyaslayın; gazın yüzeyel ülser çevresinde mi sınırlı kaldığını yoksa derin anatomik planları izleyip izlemediğini belirleyin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Penetran yabancı cisim
giriş yolunda lineer gaz ve görünür yabancı materyal bulunabilir; dağılım yaralanma traktıyla sınırlıysa derin enfeksiyon kanıtı değildir.
Amputasyon veya yakın işlem sonrası cerrahi saha çevresindeki hava, iyatrojenik doku planı girişine bağlı olabilir.
Selülitte cilt kalınlaşması, cilt altı ödemi ve yağ dokusunda çizgilenme görülebilir; intravenöz kontrast sonrası diffüz kontrastlanma veya kontrastlanan retiküler septasyonlar bland ödemden ayırmaya yardımcı olur.

Tuzaklar

  • Yumuşak doku gazı yokluğu nekrotizan fasiiti dışlamaz; gaz geç bulgu olabilir ve bazı ciddi enfeksiyonlar gaz üretmez.
