Özet
Radial baş kırığı genellikle el üzerine düşme sonrası aksiyel yükün radiokapitellar ekleme iletilmesiyle oluşur ve erişkin dirsek travmasının sık kemik yaralanmalarındandır. Kontrastsız BT kırık hattının eklem yüzündeki yerini, basamaklanmayı, fragman sayısını ve boyun uzanımını grafiden daha iyi göstererek kırık geometrisinin tarifini tamamlar. Koronoid kırığı, dirsek çıkığı veya önkol boyunca interosseöz bağ yaralanması eşlik ederse izole kırık görünümü instabiliteyi açıklamayabilir. Eşlikçi yaralanmaların atlanması dirsek ve önkol bütünlüğünün yanlış değerlendirilmesine yol açar.

- Kontrastsız ince kesitlerde radial başın subkondral eklem yüzündeki kırık hattı, basamak, impaksiyon ve fragmanlar değerlendirilir. Kemik algoritmalı aksiyel, koronal ve sagittal rekonstrüksiyonlar kırığın çevresel yerini, deplasmanını, boyun uzanımını ve proksimal radioulnar eklem ilişkisini gösterir; eşlik eden eklem yaralanmaları ayrıca taranır.
Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).
BT bulguları
- Eklem yüzü kırık hattı — radial başın subkondral plağından geçerek artiküler yüzü bir veya daha fazla parçaya ayırır.
- Eklem basamağı ve aralık — çökmüş yüzey ya da ayrılmış fragman radiokapitellar temas alanında kontur uyumsuzluğu oluşturur.
- Fragman sayısı ve dağılımı — çevresel konumlarını, ana parçayla kortikal temasını ve eklem içine dönüp dönmediğini belirtin.
- Boyun uzanımı — kırık radyal başın metafizine ve boyun korteksine devam edebilir.
- Parçalı baş kırığı — eklem yüzü çoklu fragmanlara ayrılır ve tek parça hâlindeki baş konturu kaybolur.
- Proksimal radioulnar uyum — radial başın ulnadaki radial çentikle ilişkisi korunmuş mu, subluksasyon var mı inceleyin.
- Bağlantılı dirsek yaralanması — koronoid kırığı ve ulnohumeral çıkık bulguları radial başla beraber aranır.
- Önkol boyu instabilitesi şüphesi — radius-ulna uzunluk ilişkisini ve distal radioulnar eklemi klinik bağlamla birlikte inceleyin.
Ölçütler ve sınıflamalar
- Modifiye Mason tip I/II ayrımında deplasman
- >2 mm
- Modifiye Mason sınıflaması (Hotchkiss tip II; Johnston tip IV eklemesi)
- Tip I minimal deplase kırığı; Tip II deplase marjinal kırığı; Tip III parçalı radial baş kırığını tanımlar. Johnston eklemesinde Tip IV, radial baş kırığına dirsek çıkığının eşlik etmesidir; kullanılan modifikasyon belirtilmelidir.
Normalde
Normal radial baş, radius boynunun ucunda düzgün yuvarlak eklem yüzüyle kapitelluma oturur; proksimal radioulnar eklemde ulnanın radial çentiğiyle uyumu sürer. Karşı dirseğin aynı aksiyel ve koronal seviyesinde subkondral konturu ve baş-boyun geçişini kıyaslayın; travma tarafında kesinti, basamak, çökme veya eklem dışına taşmış parça arayın.

- Radial boyun kırığı
- kırık hattı eklem yüzünden çok boyun metafizinde belirgindir; baş konturu ayrı bir eklem fragmanı şeklinde bölünmeyebilir.
- Kapitellum koronal shear kırığı
- donör defekt distal humerus eklem yüzündedir; radial baş korteksi sağlam olabilir.
- Osteokondral kapitellum lezyonu
- subkondral defekt humeral kapitellumdadır ve radial baş eklem yüzü korunmuştur.
- Koronoid kırığı
- fragman ulnanın ön çıkıntısında yer alır; radiokapitellar eklem başı ayrı bir kırık gösterebilir veya sağlam olabilir.
- Proksimal radioulnar çıkık
- radial baş ulnanın radial çentiğine göre yer değiştirmiştir; başta belirgin kırık hattı bulunmayabilir.
Tuzaklar
- Radial başın çevresel kırık yerini tek kesitten çıkarmayın; aksiyel kesitlerde başı tarayıp fragman dağılımını koronal düzlemle doğrulayın.
- Mason sınıflaması deplasman ve parçalanmayı sadeleştirir; CT raporunda eklem basamağını, fragman sayısını ve ilişkili yaralanmaları ayrıca tarif edin.
- Dirsek çıkığı eşlik eden radial baş kırığını gizleyebilir; redüksiyon sonrası kemik hizalanması normal görünse bile baş ve koronoid yüzlerini inceleyin.
- Radius başı kırığını izole dirsek yaralanması saymayın; önkol uzunluğu ve distal radioulnar eklemde uyumsuzluk varsa interosseöz bağ kompleksi hasarı olasılığını bildirin.
