Özet

Kalça septik artriti, eklem boşluğunun enfeksiyonudur ve özellikle çocuklarda eklem kıkırdağı ile femur başını hızla zedeleyebilen acil bir tablodur. Derin yerleşimli kalça ekleminde fizik muayene ve radyografi erken bulguları göstermeyebilir; ultrason efüzyonu saptamada, MR çevre kemik ve yumuşak doku yayılımını değerlendirmede öne çıkar. Erken görüntülemede eklem efüzyonu ve eşlik eden osteomiyelit saptanabilir; kesin tanı klinik değerlendirme, laboratuvar ve eklem aspirasyonu ile konur, görüntüleme tek başına etkeni doğrulamaz veya erken enfeksiyonu dışlamaz. Tanının gecikmesi kalıcı femur başı hasarı, eklem destrüksiyonu ve çevre kemik enfeksiyonuna neden olabilir.

Faz ve pencere

Kontrastsız tanısal
Bilgisayarlı tomografi eklem efüzyonu ve kapsüler distansiyonu gösterebilir; ileri vakalarda femur başı veya asetabulumda kemik erozyonu ve kortikal hasar saptanabilir, ancak erken tanı ve etken belirlemede ultrasonografi ve MRG önceliklidir. Eklem efüzyonu tek başına enfeksiyonu kanıtlamaz; erken dönem enfeksiyonlarda görüntüleme bulguları normal seydebilir ve tanıyı kesin olarak dışlamaz.
Kemik algoritması tanısal
Bilgisayarlı tomografi, kemik detaylarını ve yumuşak doku ilişkisini değerlendirmekte kullanılır; kesin tanı ve etken belirleme için ise eklem aspirasyonu ve laboratuvar incelemeleri gereklidir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).

BT bulguları

  • Eklem kapsülü distansiyonu — kapsülün genişlemesi ve eklem çevresindeki sıvı derin kalça efüzyonunu düşündürür.
  • Eklem efüzyonu — sıvı dansitesi femur başı ile asetabulum arasındaki eklem boşluğunu ve kapsül girintilerini doldurur; özgül değildir.
  • Enfeksiyon çevresinde yumuşak doku şişliği, ödem veya hiperemi bulunabilir; kesin ayırıcı tanı için laboratuvar ve aspirasyon sonuçları esas alınır.
  • İleri enfeksiyonlarda femur başı veya asetabulumda kemik yapısında bozulma ve radyolüsensi değişimler görülebilir.
  • İleri enfeksiyonlarda femur başı ve asetabulumda radyolüsensi kemik değişimleri veya kemik hasarı görülebilir.
  • Eklem boşluğunda veya çevresinde efüzyon ve içi tortulu sıvı birikebilir; bu durum ultrasonografi ve bilgisayarlı tomografi ile saptanabilir.
  • Septik artrite eşlik eden osteomiyelit, femur boynu, başı veya asetabulumda kemik iliği ve korteks tutulumu oluşturabilir.

Normalde

Karşı tarafla kıyaslandığında normal kalçada belirgin efüzyon, kapsüler genişleme veya kemik deformitesi bulunmaz. Tanıda karşı tarafın normal anatomisi ile hastalıklı tarafın efüzyon, şişlik veya kemik bulguları karşılaştırılarak ayrım yapılmalıdır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Rapidly destructive hip hastalığı
Hızla ilerleyerek femur başı ve asetabulumda ciddi kemik yıkımına yol açan nadir bir klinik tablodur.
Geçici sinovit ve inflamatuar artrit de eklem efüzyonu oluşturabilir; enfeksiyon ile ayrım için laboratuvar parametreleri ve klinik tablo değerlendirilir.
Femur başı osteonekrozunda kemik nekrozu ve yapısal bozulma ön plandadır; enfeksiyon benzeri akut kapsüler genişleme veya çevre inflamasyon tipik değildir.
Travma öyküsü olan olgularda kırık veya hemartroz düşünülmeli; enfeksiyon şüphesi yüksekse ateş, sistemik tutulum ve laboratuvar bulguları ayrımı yönlendirir.

Tuzaklar

  • Efüzyon septik artrit dışında geçici sinovit gibi durumlarda da görülebilir; tek başına enfeksiyonu doğrulamaz.
  • Erken evrede görüntülemede belirgin kemik değişikliği olmayabilir; negatif inceleme klinik şüpheli bırakır ve ek değerlendirme gerektirir.
  • Çocuk femur başı ossifikasyon çekirdeğinin düzensizliği yaşa bağlı olabilir; karşı taraf ve yaşa uygun kemikleşme görünümüyle kıyasla.
  • Yumuşak doku tutulumu, apse varlığı ve hastalık yayılımının değerlendirilmesinde manyetik rezonans görüntüleme veya ultrasonografi tercih edilir.
