Konvansiyonel osteosarkom, sıklıkla uzun kemik metafizinde gelişen ve tümör hücrelerinin osteoid ürettiği yüksek dereceli primer kemik malignitesidir. Kontrastsız BT, mineralize osteoid matriksi, kortikal destrüksiyonu ve patolojik kırığı radyografiden daha ayrıntılı gösterebilir. BT ayrıca kemik anatomisini ve tümörün mineralize bölümünü haritalar; lokal kemik iliği ve yumuşak doku yayılımının tam evrelemesinde MR gerekir. Agresif lezyonun fark edilmemesi kırık, eklem çevresi yayılım ve uygun tümör merkezi değerlendirmesinin gecikmesine yol açabilir.
Okuma sırası
- Distal femur metafizi. Koronal kemik penceresinde distal femur metafizini bulun; medüller kanal içindeki anormal yoğunluk ve litik alanların metafizden diafize doğru nasıl yayıldığını izleyin.
- Tümör osteoidi. Yoğunlukların şekline odaklanın: düzensiz bulutumsu mineralizasyon tümör osteoidini düşündürür; matriksin medulla içindeki sınırını çizin.
- Kortikal atılım. Korteks boyunca tümörün dışarı çıktığı noktayı ve ekstraosseöz mineralize kitlenin hacmini belirleyin; bu bileşeni yumuşak doku penceresinde de kontrol edin.
- Periostal yeni kemik. Korteks dışındaki ışınsal periostal kemik çıkıntılarını tarayın; keskin spiküller veya periostun üçgen biçiminde kalkması agresif yanıtı destekler.
- Kırık ve eklem yüzü. Aynı rekonstrüksiyonda kırık hattını, epifiz-subkondral kemiği ve diz eklemine komşu yumuşak dokuları gözden geçirin; kırık veya eklem çevresi uzanımı varsa sonuca ekleyin.

- Kontrastsız tanısal
- Metafizde medüller litik-sklerotik veya yoğun sklerotik destrüktif kitle; bulutumsu, amorf tümör osteoidi, kortikal kesinti ve mineralize yumuşak doku komponenti seçilir. İyotlu kontrast, osteoid matriksin tanınması için gerekli değildir.
- Kemik algoritması tanısal
- İnce kesit aksiyel görüntüler ve çok düzlemli rekonstrüksiyonlar tümör matriksinin korteks içinden dışarı uzanmasını, periostal yeni kemik oluşumunu, eklem yüzüne yaklaşımı ve kırık hattını gösterir; osteoidin küçük mineralize odakları kemik penceresinde belirginleşir.

BT bulguları
- Metafizer medüller destrüksiyon — geniş geçiş zonu, permeatif litik alan ve düzensiz skleroz aynı lezyonda birleşebilir.
- Bulutumsu tümör osteoidi — mineralize matriks medulla içinde amorf yoğunluklar oluşturur ve yumuşak dokuya doğru uzanabilir.
- Kortikal yıkım — medüller kitle korteksin bir bölümünü inceltir, keser veya dış konturu bozar.
- Mineralize ekstraosseöz komponent — korteks dışındaki yumuşak doku kitlesinde yoğun osteoid odakları görülür.
- Agresif periostal kemik — kortekse dik spiküller veya kaldırılmış periost kenarı sunburst/Codman üçgeni görünümünü oluşturabilir.
- Patolojik kırık — destrüktif metafizden geçen keskin kortikal çizgi veya parçalanma aranır.
- Eklem komşuluğu — lezyonun subkondral kemiğe, epifize veya eklem çevresi yumuşak dokuya uzanımı haritalanır.
Normalde
Normal uzun kemik metafizinde medüller trabeküller ve kortikal halka süreklidir; bulutumsu osteoid, kortikal yıkım ya da korteksten dışarı uzanan mineralize kitle yoktur. Karşı taraf metafizle korteks kalınlığını ve trabekül desenini kıyaslayın; distal femur veya proksimal tibia lezyonunda subkondral kemik ve eklem yüzünün korunup korunmadığını ayrıca izleyin.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

- Ewing sarkomu
- çocuk ve ergenlerde görülen, kemik yıkımı, kortikal branşman ve periostal reaksiyonlar yapan agresif küçük yuvarlak hücreli bir sarkomdur; osteoid matriks üretimi karakteristik değildir.
- Telanjiektatik osteosarkom
- baskın litik ve kan dolu boşluklu görünüm, konvansiyonel tipin yoğun osteoid üretiminden ayrılır.
- Osteomiyelit
- özellikle kemik tüberkülozu geniş kemik yıkımı ve kistik değişikliklere yol açabilir; soğuk apse ve sistemik enfeksiyon belirtileri eşlik edebilir, neoplastik osteoid görülmez.
- Kondrosarkom
- mineralizasyonu lobüle halka-yay biçimindedir; osteosarkomdaki amorf bulutumsu osteoid görünümünden farklıdır.
- Kemik lenfomasında diffüz kemik iliği infiltrasyonu ve yumuşak doku uzanımı görülebilir; kas-iskelet sistemi T-hücreli lenfomalarında korteksin görece korunması önemli bir görüntüleme ipucudur.
Tuzaklar
- Radyografide litik görünen tümör içindeki küçük osteoid odakları BT'de ortaya çıkabilir; tümörün tamamını tek tip litik kabul etmeyin.
- Düzensiz mineralizasyonu olgun kemikleşmiş benign bir lezyon gibi yorumlamayın; kortikal atılım ve agresif periostal yanıtla beraber değerlendirin.
- Eklem komşuluğundaki tümör epifizde de görülebilir; kemik içi devamlılığı izleyip eklem kıkırdağını tümör uzanımından ayırın.
- BT yumuşak doku ve kemik iliği uzanımının tüm sınırlarını belirlemede MR'ın yerini tutmaz; lokal evreleme için MR korelasyonunu belirtin.
