Özet

Vertebra metastazı epidural aralığa ilerlediğinde omurilik, kauda ekuina veya sinir köklerini sıkıştırabilir; yeni güç kaybı ya da sfinkter bozukluğu bu nedenle zaman duyarlı bir bulgudur. BT hızlı çekim ve kortikal bütünlüğü değerlendirmede yararlıdır; geç dönemde epidural yumuşak doku kitlesi ve komşu kemik yıkımı görülebilir, ancak erken epidural metastazı saptamadaki rolü sınırlıdır. Kanalın tümörle daraldığı kuşkusunda MR, epidural uzanımın ve nöral basının kapsamını belirleyen incelemedir. Yayılımın erken tanınıp müdahale edilmesi, nörolojik işlevin korunmasına ve vertebral kolonun stabilizasyonuna katkı sağlayabilir.

Okuma sırası

  1. Vertebra gövdelerinin posterior korteks hattı. Sagittal kemik seride vertebral gövdelerin arka korteksini kraniokaudal yönde izleyin; kortikal çizginin bozulduğu ve gövde yüksekliğinin azaldığı metastatik seviyede aksiyel kesitlere geçin.
  2. Ventral epidural aralık. Aksiyel yumuşak doku kesitinde şüpheli vertebranın hemen önündeki kanal bölümünü açın; epidural yağın tek tarafta silinip silinmediğini ve bunun kemik odağına bağlanıp bağlanmadığını takip edin.

Kontrastsız tanısal

BT kortikal bütünlüğü değerlendirebilir ve geç dönem epidural metastazları gösterebilir; erken epidural metastazları saptamadaki rolü sınırlıdır.

BT bulguları

  • Posterior korteks aşılması — vertebra gövdesindeki tümör odağından kanala doğru uzanan, kemik korteksi gibi keskin sınırlanmayan yumuşak doku bileşeni.
  • Epidural yağ silinmesi — normalde vertebra ile dural kese arasında seçilen yağ planının fokal olarak kaybolması veya asimetrik daralması.
  • Epidural yumuşak doku — çoğunlukla ventral kanalda, vertebral kemik odağıyla bitişik veya devamlılık gösteren kitle dansitesi.
  • Kanal konturunda bası — epidural bileşenin dural kese alanını içe doğru itmesi; BT görünümü kordun gerçek bası derecesini belirlemez.

  • Bilsky ESCC ölçeğinde derece 3, kord basısına eşlik eden BOS çevresinin tamamen kaybolduğu durumu ifade eder. Metastatik epidural kord basısının değerlendirilmesinde MR tercih edilir; BT kemik korteksini değerlendirmede yararlıdır ve MR’ın yumuşak doku ile kord basısı değerlendirmesinin yerini tutmaz.

Ölçütler ve sınıflamalar

Bilsky epidural spinal kord basısı ölçeği (ESCC)
Ölçek aksiyel T2 ağırlıklı MR görüntüsünde en fazla bası olan düzeyden değerlendirilir. Bilsky ESCC derece 3, kord basısının BOS mesafesini tamamen ortadan kaldırdığı durumu ifade eder. Rutin BT, kord ve BOS sınırlarını bu dereceleri güvenle atayacak biçimde göstermez.

Normalde

Normal epidural aralık epidural yağ içerir; epidural lezyon MR'da bu yağ planını silebilir ve dural keseyi içe doğru yer değiştirebilir. Şüpheli seviyede BT ile kemik korteksinin bütünlüğünü, MR ile epidural ve transforaminal tümör uzanımını değerlendirin.
