Özet

Faset osteoartriti, özellikle alt lomber düzeylerde ve yaşla birlikte sık görülen dejeneratif eklem hastalığıdır. BT, faset osteoartritinde kemik ayrıntısını iyi gösterir; eklem aralığı daralması, artiküler proses hipertrofisi ve osteofitler ile subkondral skleroz ve kistler değerlendirilebilir. BT'de görülen faset dejenerasyonu tek başına ağrının kaynağını doğrulamaz. İleri hipertrofi lateral reses veya forameni daraltabilir ve eşlik eden kök basısını gözden kaçırmak klinik korelasyonu bozabilir.

Kontrastsız tanısal
İnce kesit kemik algoritmasında eklem aralığı daralması, eklem yüzlerinin sklerozu, subkondral kistler ve marjinal osteofitler seçilir; aksiyel görüntüler oblik eklem hattına göre, sagittal ve koronal MPR ile tamamlanır.

Normalde

Normal faset ekleminin karşılıklı kortikal yüzleri düzgün, aralığı seçilebilir ve artiküler prosesleri birbirine göre belirgin hipertrofik değildir. Aynı düzeyde sağ ve sol eklemleri, kraniyal ve kaudal komşu fasetleri karşılaştır; aralık kaybının yanı sıra skleroz, kist ve osteofitin birlikte bulunup bulunmadığına bak.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Lomber faset eklemleri aksiyel kesitte eğik seyreder; tek kesitte eklem aralığının görünmemesi gerçek ankiloz veya ileri daralma sanılabilir, ardışık kesit ve oblik MPR kullan.
  • Faset eklem gazı her zaman osteoartrit derecesini göstermez; gazı osteofit, skleroz ve aralık kaybıyla birlikte yorumla.
  • Kemik penceresindeki hipertrofi santral kanal darlığını olduğundan belirgin gösterebilir; kanal ve reses lümenini yumuşak doku penceresinde yeniden kontrol et.
  • BT’de faset osteoartritinin görülmesi ağrının faset ekleminden kaynaklandığını tek başına kanıtlamaz; görüntüleme bulguları özgül klinik bulgularla birlikte yorumlanmalıdır.

    Faset osteoartriti. Birlikte bulunan eklem aralığı kaybı, subkondral skleroz ve marjinal osteofitler dejeneratif faset artrozunu destekler. Ayırıcı tanıda septik faset artriti radyolojik taklitçilerle karışabilen nadir ve ciddi bir enfeksiyondur; pars defekti pars interartikülarisin kendine özgü kırığıdır, travmatik faset bozukluklarında ise yer değiştirme (çıkık) ön plandadır.

  2. Faset osteofiti lateral resese uzanıyor. Bu bulgunun doğrudan önemi hangi yapıyla ilişkilidir?
