Özet
Multipl miyelom plazma hücrelerinin kemik iliği hastalığıdır ve omurgada fokal litik odaklar, yaygın kemik kaybı veya patolojik kompresyon kırığıyla ortaya çıkabilir. Kontrastsız düşük doz tüm vücut BT, kortikal ve trabeküler kemik yıkımını hızlı biçimde gösterir; sagittal rekonstrüksiyon korpus çökmesini ve kanal yönündeki kemik parçasını ortaya koyar. BT'de litik destrüksiyon olmadan yalnız kemik iliğinde hastalık bulunabileceği için negatif kemik BT'si iliği temiz kabul ettirmez. Lezyon ya da kırık gözden kaçarsa hastalık yükü ve vertebral mekanik/nöral risk eksik değerlendirilebilir.

- Düşük doz tüm vücut incelemede kemik algoritmalı aksiyel kesitler keskin sınırlı, çoğunlukla sklerotik kenarı olmayan trabeküler litik odakları; sagittal rekonstrüksiyon korpus yüksekliği kaybı, vertebra plana ve retropulsiyonu gösterir. İyotlu kontrast kemik hastalığının saptanması için gerekmez.
- Kemik algoritması tanısal
- Aksiyel ve sagittal ince kemik rekonstrüksiyonlarında trabeküler destrüksiyon, kortikal devamlılık, pedikül tutulumu, çökme derecesi ve posterior duvarın kanal içi konumu değerlendirilir.
Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).
BT bulguları
- Punched-out litik odak — trabeküler kemikte keskin sınırlı, çoğu kez sklerotik kenarı olmayan fokal destrüksiyon.
- Yaygın osteolitik tutulum — birden çok vertebra korpusunda trabeküler ağ seyrelir veya silinir; eşlik eden kortikal incelme aranır.
- Vertebra plana — korpus yüksekliği ileri derecede azalır; komşu seviyelerle kıyaslayarak yeni veya eski çökme belirlenir.
- Malign kırık işaretleri — korpus içi destrüksiyona korteks, pedikül ya da posterior eleman kaybı eşlik edebilir.
- Posterior duvar retropulsiyonu — kemik fragmanı kanala doğru yer değiştirerek kanal çapını daraltabilir.
- Epidural veya paravertebral kitle — yumuşak doku bileşeni kırık ya da litik korpusun ötesinde spinal kanal veya paravertebral alana uzanır.

Normalde
Normal korpusta trabeküler kemik ağı devamlıdır, kortikal kabuk ve posterior duvar kesintisizdir; korpus yüksekliği komşu seviyelerle uyumludur. Şüpheli trabeküler litik odakları belirgin osteoporozdaki seyrelmiş trabeküllerden ayırmaya çalışın; vertebral çökme ve retropulse fragman için sagittal MPR'leri inceleyin.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

- Enfeksiyöz spondilodiskitte tipik olarak komşu vertebra gövdeleri ve karşılıklı son plaklar yıkılır, aradaki disk de tutulur; bu bulgular ayırıcı tanıda dikkate alınmalıdır.
- Hiperparatiroidi, yaygın osteoporoz ve çoklu litik lezyonlarla miyelom ayırıcı tanısında yer alabilir.
Tuzaklar
- Osteoporozdaki seyrek trabekülleri küçük litik odak sanmayın; şüpheli alanı ardışık ince kesitlerde ve diğer vertebralarla karşılaştırın.
- BT'de kortikal destrüksiyon görülmemesi iliğe sınırlı miyelomu dışlamaz; BT'nin kemik iliği infiltrasyonuna duyarlılığının sınırlı olduğunu rapor yorumunda koruyun.
- Korpus çökmesini tek başına miyelom kırığı saymayın; osteoporotik kırıkta intravertebral vakum, retropulse fragman ve korunmuş pediküller benign lehine olabilir.
- Rutin gövde BT'si tüm iskeleti kapsamayabilir; omurga bulgusu sistemik hastalık yükünün tam haritası olarak sunulmamalıdır.

Miyelomla uyumlu osteolitik lezyon. 8 mm'lik destrüktif litik odak önerilen 5 mm eşiğini aşar ve miyelom kemik hastalığıyla uyumludur. Vertebra hemanjiomlarında dikey trabekül kalınlaşması polka-dot veya corduroy benzeri paternler oluştururken, miyelomda belirgin trabeküler yıkım ve sklerotik yanıt yokluğu görülür; ayrıca küçük hipodens lezyonlar yağlı marros içeriği nedeniyle yanlış pozitif olarak değerlendirilebilir.
Miyelom şüphesi olan hastanın BT'sinde vertebra korteksleri sağlam ve litik odak yok. Hangi çıkarım doğrudur?
