Özet

Humerus cisim kırığı, proksimal ve distal metafizler arasında kalan diyafiz korteksinin travma veya zayıflamış kemik zemini üzerinde sürekliliğini kaybetmesidir. Çoğu olguda iki düzlemli radyografi tanı için yeterlidir; BT, kırığın omuz ya da dirsek eklemine uzanımının belirsiz olduğu, parçalanmanın karmaşık olduğu veya eşlik eden kemik lezyonunun haritalanması gereken durumlarda kemik geometrisini netleştirir. Kırık yönü, deplasman, açılanma ve parçalı segmentler ile medüller/kortikal lezyon birlikte değerlendirilir. Kırığın eklem uzanımının veya patolojik zeminin atlanması yaralanma kapsamını eksik bırakır; radial sinir ve brakiyal damar komşuluğu ayrıca klinik muayene bağlamında ele alınır.

Okuma sırası

  1. Humerus diyafiz korteksi. Kırık hattını inceleyin; özellikle oblik veya parçalı örüntülerde proksimale ya da distale uzanan ince hatları arayın.
  2. Ana kırık fragmanları. Proksimal ve distal ana fragmanların yönünü izleyin; açılanma ve ötelemenin hangi düzlemde belirgin olduğunu karşılaştırın.
  3. Ara fragmanlar ve medüller kanal. Kırık aralığındaki ara parçaları ve medüller teması inceleyerek basit çizgi ile parçalı örüntüyü ayırın.
  4. Eklem yüzleri ve kırık çevresi kemik. Son olarak kırık hattının omuz/dirsek eklemine uzanımını ve çevresinde tümör düşündüren odak ya da yumuşak doku kitlesi bulunup bulunmadığını tamamlayın.

Faz ve pencere

Kontrastsız tanısal
Kortikal kesinti, kırık hattının oblik/transvers/spiral yönelimi, fragmanların öteleme ve açılanması, parçalanma ile proksimal-distal eklem yüzlerine uzanım izlenir.
Kemik algoritması tanısal
İnce kesit ve uzun eksene paralel koronal-sagittal reformasyonlar kırık uçlarının rotasyonunu, kortikal temasını, küçük kelebek fragmanını ve şüpheli lezyonun korteks/medulla ile ilişkisini gösterir.

Önerilen pencereler: Kemik (G 1800 / M 400).

BT bulguları

  • Kırık yönelimi — diyafiz boyunca transvers, oblik, spiral veya segmental çizgi; hattı birden çok planda takip edin.
  • Fragman deplasmanı — başlıca proksimal ve distal parçaların öteleme, açılanma ve rotasyon yönü tarif edilir.
  • Kelebek fragmanı ve parçalanma — kompresyon/torsiyon bileşenine eşlik eden ara kortikal parçalar ve fragman sayısı gösterilir.
  • Kırık uçlarının kemik morfolojisi ve kırık örüntüsü BT’de ayrıntılı değerlendirilebilir.
  • Eklem uzanımı — proksimalde humerus başı/tüberkül veya distalde kondil-troklea eklem yüzüne ulaşan hat aranır.
  • Patolojik zemin — kırık çevresinde kortikal yıkım gibi agresif özellikler varsa ileri patolojik değerlendirme gerekir.
  • Yumuşak doku bileşeni — kırık çevresinde kortikal destrüksiyon veya yumuşak doku uzanımı varsa olguyu yalnız travmatik kırık olarak değerlendirmeyin.
  • AO/OTA sınıflamasında humerus cisim kırıkları basit (A), kama (B) ve çok parçalı (C) örüntüler olarak sınıflanır.

Humerus cismi, proksimal ve distal uç segmentler arasındaki diyafiz bölgesidir. Humerus cisim kırığının yeri, proksimal ve distal uç segmentler arasındaki şaft bölgesine göre tanımlanır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
