Özet

Proksimal humerus kırıklı çıkığı, başın glenoid dışına yer değiştirmesi ile eşlik eden kırık parçalarının değerlendirilmesinde kontrastsız BT'nin değerli olduğu kompleks bir travmatik yaralanmadır. Kontrastsız omuz BT'si, özellikle röntgenlerde belirsiz olan durumlarda veya intraartiküler parçaların varlığında, kırık desenlerinin ve eklem yüzü ilişkisinin detaylı olarak haritalandırılmasında yardımcı olur. Bu örüntüde aksiller sinir ve damar komşuluğu ile humerus başı kanlanmasını etkileyebilecek kırık özellikleri dikkatle tarif edilmelidir. Çıkık bileşeninin veya glenoid içindeki sıkışmış fragmanın atlanması eklem uyumunu ve eşlik eden yaralanmaları eksik tanımlar.

Kontrastsız tanısal
Humerus başının glenoid eklem yüzüyle ilişkisi, anterior/posterior yer değiştirme, boyun ve tüberkül kırıklarının ayrışması, baş impaksiyonu ve glenohumeral aralıktaki kemik fragmanları değerlendirilir; rutin kırık haritalaması kontrastsız yapılır.
Kemik algoritması tanısal
Çok düzlemli rekonstrüksiyonlar ve aksiyal görüntüler, kırık hattlarının yönünü, tüberkül ayrışmasını ve humerus başının glenoid ile ilişkisini karakterize etmek için kullanılır.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Glenohumeral uyumsuzluk — humerus başının korakoid sürecine göre konumu ile belirgin anterior veya posterior yer değiştirmesi; yön tayini için korakoid referansı ve aksiyal görüntüler kullanılır.
  • Boyun kırığı — Neer kriterlerine göre (≥1 cm öteleme veya ≥45° açılanma) eklem yüzünü taşıyan baş segmentinin tüberkül veya şafttan bağımsız bir parça haline gelmesi.
  • Cerrahi boyun kırığı — humerus başı ve tüberkülün şafttan bağımsız hale geldiği kırık hattının genellikle cerrahi boyun bölgesinde seyrettiği ve medial kalkar bütünlüğünün vasküler yatak açısından kritik öneme sahip olduğu desendir.
  • Tüberkül fragmanları — rotator manşetin yapışma bölgelerini oluşturarak yer değiştirebilir ve fonksiyonu bozabilir.
  • Baş impaksiyonu veya split bileşeni — eklem yüzünde çökme ya da başı iki parçaya ayıran yarık saptanır.
  • Eklem içi fragman — glenoid ile humerus başı arasında sıkışmış kemik parçası redüksiyon sonrası kongruansı bozabilir.
  • Glenoid kenar yaralanması — eşlik eden anterior veya posterior rim kırığı çıkık yönüyle birlikte raporlanır.
  • Aksiller nörovasküler komşuluk — medial deplase fragmanlar aksiller damar paketine yaklaşabilir; damar lümeni kontrastsız BT'de güvenilir biçimde açıklanmaz.

Neer sınıflaması
Proksimal humerus dört segmentle ele alınır: baş, büyük tüberkül, küçük tüberkül ve şaft. Eşik üstü yer değiştiren parçaların sayısına göre bir-, iki-, üç- veya dört-parçalı örüntü tanımlanır.
Mayo-FJD örüntü sınıflaması
Başın ciddi etkilendiği örüntülerde baş çıkığı, baş split ve ağır impaksiyon; diğer gruplarda izole tüberkül veya anatomik/cerrahi boyun kırığı örüntüleri ayrılır.

Normalde

Normal glenohumeral BT'de humerus başı glenoid fossa üzerinde korunmuş eklem yüzüyle uyumlu şekilde oturur; korakoid süreci ile olan ilişkisi bozulmamıştır ve proksimal humerusda kırık hattı izlenmez. Tüberkül fragmanlarının konum ve yer değiştirmesini BT'de dikkatle değerlendirin; büyük tüberkül kırığı postredüksiyon grafilerinde gözden kaçabilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Postredüksiyon BT'de humerus başının glenoid üzerine oturuşunu doğrularken, eşlik eden Hill-Sachs lezyonlarını, intraartiküler kemik fragmanlarını ve redüksiyonu engelleyen yapıları aktif olarak arayın.
  • Akut travma sonrasında rotator manşet, deltoid ve skapula çevresi kasların geçici atonisi, kolun ağırlığıyla humerus başında inferior subluksasyona yol açabilir.
  • Kontrastsız BT damar patolojilerini güvenilir şekilde ekarte edemez; vasküler zedelenme şüphesinde kesin tanı için kontrastlı BT anjiyografi veya konvansiyonel anjiyografi şarttır.
  • Glenohumeral aralıktaki küçük kemik fragmanlarını BT'de dikkatle inceleyin; bu fragmanlar humerus başının glenoid üzerine redüksiyonunu engelleyebilir.
