Özet

Malignite ilişkili patolojik kompresyon kırığı, tümörün vertebra korpusunu zayıflatmasıyla gelişebilir ve bilinen kanser öyküsü olmasa da litik veya infiltratif odakla ilk kez fark edilebilir. BT kemik korteksini, posterior duvarı ve kırık morfolojisini değerlendirmede yararlıdır; kemik iliği infiltrasyonu, tümör yayılımı ve epidural uzanım için MR daha duyarlıdır. BT'de kitle, ekspansiyon veya kemik iliği bulguları belirsiz kaldığında MR tümör yayılımı ve nöral yapı etkisini daha iyi değerlendirir. Maligniteyi benign osteoporotik kırık olarak yorumlamak hastalık yayılımının ve kanal içi uzanımın eksik tanınmasına yol açabilir.

Faz ve pencere

Kontrastsız tanısal
Kemik penceresinde BT ile korpus korteksi, posterior duvar ve kırık morfolojisi değerlendirilir; kemik iliği infiltrasyonu, tümör yayılımı ve epidural uzanım için MR daha duyarlıdır. Sagittal ve aksiyel kemik rekonstrüksiyonları posterior duvar, son plak ve pedikül uzanımını gösterir. Kemik morfolojisi için kontrast zorunlu değildir; MR kemik iliği, epidural yayılım ve nöral yapılar için daha duyarlıdır.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Litik destrüksiyon — osteolitik lezyonda normal trabeküler kemiğe göre atenüasyon azalması ve kemik mimarisinde yıkım görülür.
  • Kortikal yıkım — BT, vertebra korteksindeki yıkımı ve posterior duvar bozulmasını değerlendirmede yararlıdır.
  • Pedikül/arka eleman tutulumu — spinal metastazlar vertebra anatomisinde yıkıma yol açabilir; tek başına posterior eleman bulgusu maligniteyi kanıtlamaz.
  • Korpus ekspansiyonu — vertebra dış konturunun tümöral genişlemesi ve normal kortikal sınırın kabarıp incelmesi.
  • Paravertebral yumuşak doku kitlesi — vertebra komşuluğunda kemik sınırını aşan fokal solid komponent.
  • Epidural uzanım — spinal kanal içinde yumuşak doku yoğunluğu ve kanal içi alanın daralması; omurilik/kauda ekuina ilişkisi BT'de sınırlı olabilir.
  • Çok odaklı iskelet tutulumu — görüntü alanındaki diğer vertebra veya kaburgalarda benzer litik, sklerotik ya da mikst metastatik odaklar.
  • Patolojik çökme — zayıflamış korpusta yükseklik kaybı, düzensiz kırık hattı ve destrüktif odağın aynı seviyede birlikteliği.

Normalde

Komşu normal trabeküler kemikle karşılaştırarak litik odaklarda azalmış atenüasyonu arayın; ayrıca korpus ekspansiyonu ve paravertebral yumuşak doku kitlesini değerlendirin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • Posterior eleman tutulumu maligniteyi düşündürür ancak akut osteoporotik kırıkta reaktif değişiklik olabilir; kemik yıkımı, yumuşak doku kitle bulgusu ve MR ile birlikte yorumlayın.
  • Korpusun posterior sınırındaki retropulse keskin kemik fragmanını tümöral ekspansiyonla karıştırmayın; fragman kırık hattıyla ilişkili ve köşeli, tümöral kontur daha yaygın kabarıktır.
  • Yalnız litik görünüm metastaz tanısı koydurmaz; miyelom, enfeksiyon ve primer kemik lezyonları kemik yıkımını taklit edebilir.
  • Kontrastsız BT'de epidural yumuşak doku seçilmeyebilir; nörolojik bulgu veya kanal kompresyonu kuşkusunda MR ile epidural alanı değerlendirin.
