Özet
Kronik tromboembolik pulmoner hastalık (CTEPD), en az 3 ay terapötik antikoagülasyon sonrasında süren semptomlar, V/Q görüntülemede uyumsuz perfüzyon defektleri ve CTPA/DSA’da kronik organize fibrotik trombüs bulgularıyla tanımlanır; pulmoner hipertansiyon (PH) yoksa CTEPD, PH hemodinamik olarak doğrulanmışsa CTEPH terimi kullanılır. BTPA damar içi web ve bantları, duvara yapışık organize trombüsü ve damar kesilmesini doğrudan gösterirken akciğer penceresi ile perfüzyon haritası parankimal etkilenimi ortaya koyar. Mozaik atenüasyon ve sağ kalp değişiklikleri hastalığın yaygınlığını düşündürebilir, fakat mozaik patern tek başına özgül değildir. Bu bulguların atlanması düzeltilebilir kronik damar tıkanıklığının ve pulmoner hipertansiyonun tanısının gecikmesine neden olabilir.

- BTPA tanısal
- Pulmoner arter kontrastı içinde duvara geniş açıyla oturan eksantrik organize trombüs, ince intralüminal web/bant, ani stenoz, pouch biçimli kör sonlanma veya distal kontrast kesilmesi görülür. Akciğer penceresinde hipoperfüze alanlar düşük atenüasyonlu olabilir ve bu alanlarda damar kalibresi azalabilir; çift enerjili BT iyot haritalarında perfüzyon defektleri tipik olarak periferik ve kama biçimindedir.
Önerilen pencereler: Anjiyo (G 600 / M 150), Akciğer (G 1500 / M -600), Mediasten (G 350 / M 50).
BT bulguları
- Organize eksantrik trombüs — arter duvarına yapışık, lümeni hilal biçiminde daraltan ve duvarla geniş açı yapan materyal.
- Web veya bant — lümen içinde ince çizgisel dolum defekti; kısmi rekanalizasyonu yansıtır.
- Ani damar daralması ve poststenotik genişleme — stenoz sonrası kalibrasyon artışı kronik akım etkisini gösterir.
- Pouch lezyonu veya damar amputasyonu — tıkalı damarın kör, kese biçimli sonlanması ya da distal dalın kaybı.
- Damar budanması — hipoperfüze bölgede segmental ve subsegmental arterlerin azalmış kalibresi/sayısı.
- Mozaik atenüasyon — coğrafi düşük ve görece yüksek atenüasyon alanları; düşük atenüasyonlu alanda küçük damarlar daha seyrektir.
- Sistemik kollateraller — genişlemiş ve kıvrımlı bronşiyal arterler kronik damar tıkanmasına eşlik edebilir.
- Kronik sağ kalp/pulmoner hipertansiyon bulguları — sağ boşluk genişlemesi, septum düzleşmesi ve ana pulmoner arter genişlemesi.
Normalde
Normal pulmoner arterler periferde düzenli biçimde incelerek segmental dallara ayrılır; intralüminal web, ani kesilme veya duvara yapışık kalıcı dolum defekti görülmez. Normal akciğerde atenüasyon ve damar yoğunluğu lobüller arasında görece homojendir; şüpheli düşük atenüasyonlu bölgeyi komşu yüksek atenüasyonlu alanla ve aynı düzeydeki karşı akciğerle damar kalibresi açısından kıyaslayın.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

- Akut pulmoner emboli
- merkezî veya akut açı yapan dolum defekti ve korunmuş/geniş damar kalibresi görülür; web, geniş açı, damar küçülmesi ve kollateraller kronikliği destekler.
- Küçük hava yolu hastalığı
- mozaik atenüasyona ekspirasyonda hava hapsi ve bronş duvarı değişiklikleri eşlik eder; vasküler tipte düşük atenüasyon alanında damarlar seyrektir.
- Amfizem
- düşük atenüasyon alanlarında damar azlığı olabilir, ancak eşlik eden amfizematöz destrüksiyon ve hava yolu bulguları dağılımı açıklar.
- Pulmoner arterit, CTEPD'yi taklit edebilen durumlardan biridir ve ayırıcı tanıda dikkate alınmalıdır.
Tuzaklar
- Mozaik atenüasyon CTEPD'ye özgü değildir; düşük atenüasyon alanındaki damar sayısını, hava yolu hastalığı bulgularını ve varsa ekspirasyon görüntülerini birlikte değerlendirin.
- Solunum hareketi veya geçici kontrast kesintisi (TIC), CTPA'da gerçek dolum defektini taklit edebilir; bu olasılığı görüntüleri yorumlarken göz önünde bulundurun.
- Distal kronik hastalık BTPA'da gözden kaçabilir; pulmoner arter dallarını periferde izleyin ve anatomik BT normal görünse bile perfüzyon defekti olasılığını dışlamayın.
