Özet

Mastektomi, aksiller diseksiyon veya rekonstrüksiyon sonrasında seroma ve hematom cerrahi yatak ya da implant çevresinde koleksiyon oluşturabilir. Kontrastsız ve kontrastlı BT, cerrahi yatakta oluşan sıvı veya kan koleksiyonlarının atenüasyonunu, yayılımını ve komşu dokulardaki değişiklikleri değerlendirmeye yarar; postoperatif meme koleksiyonlarında ultrason genellikle ilk tercih görüntüleme yöntemidir. Hematoma içeriği zamanla heterojenleşebilir ve yoğunluğu değişebilir. Enfekte koleksiyonun veya aktif kanama bulgusunun gözden kaçması komplikasyonun kapsamını olduğundan az gösterebilir.

Okuma sırası

  1. Cerrahi yatak. Mastektomi yatağı veya aksiller diseksiyon sahasını takip ederek sıvı birikiminin nerede başladığını işaretle.
  2. İmplant kapsülü. Koleksiyon implantla ilişkiliyse protez dış konturunu izleyip sıvının kapsülün hangi bölümünde biriktiğini göster.
  3. Koleksiyon içeriği. Kontrastsız kesitte içeriğin homojen düşük atenüasyonlu mu yoksa pıhtılaşma/kanla uyumlu heterojen yüksek atenüasyonlu mu olduğuna bak.
  4. Koleksiyon duvarı ve çevresi. Son olarak duvar kalınlığını, çevre yağ dokusunu, gazı ve uygun faz varsa kontrast ekstravazasyonunu incele.

Faz ve pencere

Kontrastsız tanısal
Seroma genellikle suya yakın düşük atenüasyonlu içerik taşır; akut kanama daha yüksek atenüasyonlu olabilir, fakat hematom zamanla heterojenleşip dansite kaybedebilir.
Portal tanısal
Koleksiyon duvarında çevresel kontrastlanma, komşu yumuşak doku planlarında kirlenme ve enfeksiyon bulguları değerlendirilir; aktif kanama kuşkusunda kontrastlı görüntüleme gerekir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Mediasten (G 350 / M 50).

BT bulguları

  • Seroma mastektomi yatağında veya aksiller diseksiyon alanında düzgün sınırlı sıvı koleksiyonu olarak görülür.
  • İmplant çevresindeki sıvı koleksiyonu protezin önünde, arkasında ya da kapsül boyunca uzanabilir.
  • Akut hematom kontrastsız BT'de çevre sıvıdan daha yüksek atenüasyon gösterebilir; pıhtı ve sıvı seviyelenmesi heterojen iç görünüm oluşturabilir.
  • Subakut veya kronik kan ürünlerinde koleksiyonun atenüasyonu düşebilir; seroma ile tek kesitte ayrım güçleşebilir.
  • Enfeksiyon eklendiğinde cilt altı yağ planlarında kirlenme, cilt kalınlaşması ve kalın ya da düzensiz koleksiyon duvarı görülebilir.
  • Koleksiyon içindeki gaz, yakın zamanda girişim yoksa enfeksiyon veya fistül olasılığını artırır.
  • Arteriyel faz olmaksızın aktif kanama her zaman dışlanamaz; klinik olarak kanama kuşkusunda uygun anjiyografik protokol gerekir.

Normalde

Normal iyileşen mastektomi yatağında protez kapsülü ince ve düzenli, pektoralis ile cilt altı yağ planları belirgin koleksiyon olmadan izlenir. Aynı tarafın önceki BT'si ve karşı tarafla kıyaslamada yeni sıvı hacmi, içerik atenüasyonu, duvar kalınlığı, implant çevresindeki dağılım ve çevresel yağ dokusu değişikliği değerlendirilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

İmplant kapsül çevresi normal sıvı
az miktarda ince sıvı tek başına enfeksiyon ya da rüptür göstermez; kapsül ve protez konturu da incelenmelidir.
Aktif arteriyel kanama
koleksiyon içindeki damar yoğunluğunda kontrast ekstravazasyonu ile düşünülür; faz ve zamanlama uygun değilse BT bunu dışlayamaz.

Tuzaklar

  • Hematoma ait atenüasyon yaşla değişir; düşük dansiteli geç dönem kan ürününü otomatik seroma sayma.
  • Seroma çevresinde düzgün ince rim kontrastlanabilir; enfeksiyon lehine kalın düzensiz duvar ve çevresel inflamasyon gibi ek bulgular ara.
  • İmplant çevresinde sıvı görmek rüptür tanısı koydurmaz; protez kontur bozukluğu ve silikon yayılımı ayrıca aranır.
  • Tek portal venöz faz küçük aktif kanama odağını kaçırabilir; kanama sorusu varsa incelemenin faz yeterliliğini açıkça belirt.
