Özet

Kontrastsız beyin BT, CO zehirlenmesinde globus pallidus ve hipokampüste düşük dansiteli alanları gösterebilir. Akut incelemede iki globus pallidusun benzer biçimde düşük atenüasyon göstermesi karakteristik bir ipucudur, ancak ilk BT normal olabilir ve bu görünüm CO zehirlenmesine özgü değildir. Daha sonraki incelemelerde derin ve periventriküler beyaz cevherde yaygın hipodensite, kronik evrede ise hacim kaybı ortaya çıkabilir. CO zehirlenmesinden sonra klinik düzelmeyi izleyen bilişsel veya işlevsel kötüleşme gelişebilir; MRI, özellikle DWI, erken beyin lezyonlarını saptamada duyarlıdır.

Okuma sırası

  1. Globus palliduslar. Lentiform çekirdeklerin iç bölümündeki globus pallidusları bul; iki tarafta aynı biçimde soluklaşan odak var mı diye kesiti karşılaştır.
  2. Globus pallidus, putamen ve internal kapsül. Pallidal alanın sınırını putamen ve internal kapsüle göre çiz; hipodensitenin komşu striatuma taşıp taşmadığını belirle.

Kontrastsız tanısal

BT bulguları

  • Bilateral pallidal hipodensite — globus pallidusların iç kesimindeki simetrik düşük atenüasyon akut CO hasarının tanınan paternidir.
  • Pallidum dışı derin gri cevher tutulumu — kaudat, putamen veya talamus lezyonları pallidal değişikliğe eşlik edebilir ya da baskın olabilir.

Normalde

Her iki globus pallidusu karşılaştırın; putamen, kaudat, talamus, hipokampüs ve periventriküler ya da subkortikal beyaz cevherde eşlik eden lezyonları değerlendirin. BT erken globus pallidus hasarını göstermeyebilir; MRI bu hasarı saptamada BT’den daha duyarlıdır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
