Özet

Yaygın supratentoryal kitle etkisi, diensefalon ve mezensefalonun tentoryal incisura altından kaudal yer değiştirmesine yol açabilir. Kontrastsız BT’de sisternal CSF aralıklarının kaybı, derin orta hat yapıların kaudal yer değiştirmesi ve beyin sapı konturundaki değişim birlikte aranır. İlerleme mezensefalon-perforan damar hasarı, Duret kanaması, BOS akım engeli ve beyin sapı işlev bozukluğuyla sonuçlanabilir. Tek taraflı unkal herniasyonu santral aşağı yer değiştirmeden ayırmamak, etkilenen anatomik düzeyi ve yaygınlığı eksik tarif ettirir.

Faz ve pencere

Kontrastsız tanısal
Diensefalon ve mezensefalon kaudale yer değiştirir; perimezensefalik ve ambient sisternler iki taraflı daralır veya silinir, ileri olguda mezensefalonda uzama/deformasyon ya da küçük kanamalar görülebilir.

Önerilen pencereler: Beyin (G 80 / M 40), Subdural (G 200 / M 75).

BT bulguları

  • Diensefalonun tentoryal incisuraya göre aşağı yer değiştirmesi
  • Mezensefalonun kaudal uzaması, biçim bozulması veya orta hatta aşağı itilmesi
  • Perimezensefalik, ambient ve interpedinküler sisternlerin yaygın daralması veya silinmesi
  • Suprasellar sisternin iki taraflı basıya uğraması
  • Derin gri çekirdekler, talamus ve üçüncü ventrikül düzeyinde aşağı yönlü yer değiştirme
  • Aqueductus cerebri basısı ve eşlik eden ventriküler akım bozukluğu
  • Beyin sapında Duret kanaması veya basıya bağlı fokal hipodens hasar

Normalde

Normal aksiyel bazal kesitte interpedinküler ve ambient sisternler mezensefalonu çevreleyen açık CSF aralıklarıdır; mezensefalon ve diensefalon kesitler arasında beklenen anatomik seviyede kalır. Kaudal düzeylerde bu sistern açıklıklarının iki taraflı kaybını, orta beyin konturunu ve üçüncü ventrikül/aqueductus ilişkisini normal taraf ve komşu kesitlerle kıyasla.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

  • Bilateral perimezensefalik sisternlerin silinmesi basınç artışı göstergesidir; santral aşağı herniasyon tanısı için diensefalon ve mezensefalonun tentoryal incisuraya göre kaudal yer değiştirmesinin ardışık kesitlerde gösterilmesi gerekir.
  • Posterior fossadaki beam-hardening artefaktı prepontin ve perimezensefalik sisternleri yalancı silik gösterebilir; ardışık ince kesitler ve karşı taraf simetrisiyle kontrol et.
  • Santral herniasyon gelişen hastalarda beyin sapında (mezensefalon veya pons) yeni ortaya çıkan fokal hiperdensiteralar Duret kanaması açısından dikkatle incelenmelidir.
  • Orta hat yapısının (septum pellucidum, pineal cisim) yatay şifti tek başına transtentoryal herniasyonu tanımlamaz; herniasyon derecesini değerlendirmek için diensefalik yapıların konumu da incelenmelidir.
