Özet

Mediastinal germ hücreli tümörler çocuklukta nadirdir ancak özellikle ön mediastende yerleşir; olgun teratom bebeklik ve küçük çocuklarda, malign alt tipler ise ergenlikte ayırıcı tanıda önem kazanır. BT, kitle içindeki yağ, sıvı, yumuşak doku ve kalsifikasyonu göstererek olgun teratomu düşündürebilir ve damar/perikard ilişkisini haritalar. Olgun olmayan veya malign alt tiplerde büyük, heterojen kitleler, çevre dokulara invazyon, nekroz ve hemoraji bulguları daha agresif biyolojik davranış olasılığını düşündürür; kesin tanı için doku örneği gereklidir. Kitle etkisini veya malign yayılımı atlamak, solunum-dolaşım basısını ve hastalığın kapsamını eksik tarif etmeye yol açar.

Faz ve pencere

Kontrastsız tanısal
Makroskopik yağ düşük atenüasyonlu alanlar halinde, kalsifikasyon/kemik yüksek atenüasyonlu odaklar halinde; sıvı ve solid bileşenlerden ayrılır. Yağ-sıvı-yumuşak doku-kalsiyum birlikteliğini saptamak için kontrastsız seri tanısaldır.
Kontrastlı tanısal
Kontrastlı toraks BT solid bileşenlerin kontrastlanmasını, nekrotik veya kistik alanları ve tümörün komşu yapılarla ilişkisini net biçimde gösterir; boyut, yerleşim, yoğunluk ve lezyon içi özellikleri değerlendirmede tanısaldır.

Önerilen pencereler: Mediasten (G 350 / M 50), Yumuşak doku (G 400 / M 40), Akciğer (G 1500 / M -600), Kemik (G 1800 / M 400).

BT bulguları

  • Prevascular merkez — sternum arkasındaki ön mediastinal kitleyi ve timus yatağıyla ilişkisini tanımlayın.
  • Makroskopik yağ — kitle içindeki düşük atenüasyonlu yağ odaklarını çevre mediastinal yağla karıştırmadan gösterin.
  • Sıvı/kistik bileşen — düzgün sınırlı düşük atenüasyonlu alanların solid doku bileşenleri ve varsa ince kapsül veya septasyonlarla ilişkisini inceleyin.
  • Kalsifikasyon veya diş/kemik benzeri yoğunluk — olgun teratom içinde kaba, diş benzeri veya kemiksi odaklar görülebilir.
  • Karışık doku matriksi — yağ, sıvı, solid yumuşak doku ve kalsiyum bileşenlerinin birlikte bulunması olgun teratom için karakteristiktir.
  • İnvaziv solid doku — düzensiz sınır, damar/perikard teması, nekroz veya çevre dokuda bozulma malign alt tip olasılığını artırır.
  • Torasik yayılım — plevral sıvı/nodül, akciğer lezyonları, mediastinal nodlar ve karşı kompartımana uzanımı tarayın.

Ölçütler ve sınıflamalar

Çocukluk çağı mediastinal germ hücreli tümörleri
WHO sınıflamasına göre çocukluk çağı mediastinal germ hücreli tümörleri klinik ve biyolojik özelliklerine göre prepubertal ve postpubertal tiplere ayrılır; histolojik olarak seminom ve nonseminomatöz tümörler (olgun ve immatür teratom, vitellus kesesi tümörü, embriyonel karsinom, koryokarsinom ve karma tümör) olarak sınıflandırılır; kesin tanı doku örneğiyle histopatolojik inceleme gerektirir. Çocuklarda prepubertal ve postpubertal biyolojik gruplar da ayırt edilir. BT bileşenleri düşündürebilir ancak histolojik alt tipi belirlemez.

Normalde

Normal ön mediastende timus ve yağ dokusu görülür; içinde kitleye ait yuvarlak yağ cepleri, sıvı lokülleri veya diş/kemik yoğunluğunda odaklar bulunmaz. Aynı düzeyde perikard, brakiosefalik venler ve SVC'nin doğal konturunu referans alıp kitlenin bu yapıları itip itmediğini, sarıp sarmadığını ya da sınırlarını bozup bozmadığını kıyaslayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Lenfoma
çocuklarda T-lenfoblastik lenfoma çoğu kez tek, büyük ön mediastinal kitle olarak ortaya çıkabilir; başka alt tiplerde birden fazla nodal istasyon tutulabilir. Lenfoma — ön mediastinde geniş, homojen yumuşak doku kitlesi olarak ortaya çıkar; teratomun ayırt edici özelliği olan makroskopik yağ ve diş benzeri kaba kalsifikasyonlar lenfomada görülmez.
Timik rebound
düzgün bilobüle homojen doku damarları sarmadan genişler; karışık kistik-yağlı-kalsifik kitle oluşturmaz.
Substernal tiroid
boyundaki bezle devamlılık ve belirgin kontrastlanma gösterir; teratom içi yağ-sıvı-kalsiyum kombinasyonu yoktur.
Lenfatik malformasyon
çok odacıklı sıvı yoğunluklu lezyon ve ince septalarla seyreder; kaba kalsifikasyon ve yağlı solid matriks tipik değildir.
Timik kist
tek veya multiloküle sıvı içerikli odak olarak görünür; yağ ve kemiksi yoğunluk içeren karışık matriks beklenmez.

Tuzaklar

  • Olgun teratomda her bileşen bir arada olmayabilir; yağ veya kalsifikasyon görülmemesi teratomu tek başına dışlamaz.
  • Kontrastlı kesitte kısmi hacim etkisiyle düşük atenüasyonlu solid doku yağ sanılabilir; kontrastsız seride aynı odağı ve komşu yağ planını kontrol edin.
  • Teratomun kist içeriği homojen sıvıdan farklı atenüasyon gösterebilir; yoğun içerik tek başına solid tümör parçası demek değildir.
  • Büyük kitle orta hatta yayılabilir; yalnız çevrelediği kompartımana bakarak başlangıç yerini değil, en geniş hacmin ve timus yatağının konumunu kullanın.

  2. Ergen hastada solid ön mediastinal kitle, düzensiz perikard sınırı ve yaşa göre belirgin yüksek serum AFP var; belirgin yağ-kalsiyum birlikteliği yok. Hangi grup daha çok düşünülmelidir?

    Malign nonseminomatöz germ hücreli tümör. Belirgin AFP yüksekliği, özellikle vitellus kesesi bileşeni içerebilen malign nonseminomatöz germ hücreli tümör grubunu destekler. Görüntüleme invazyon ve kitle bileşenlerini gösterir; histolojik alt tip doku tanısıyla belirlenir. Normal/rebound timus düzgün ve homojen, basit timik kist ise sıvı içeriklidir.
