Özet
Peritonsiller apse genellikle akut tonsillitin bir komplikasyonu olarak gelişir ve peritonsiller boşlukta yer alır. Kontrastlı boyun BT'si, tonsil çevresindeki koleksiyonu ve parafarengeal uzanımı göstererek enfeksiyonun yayılımını değerlendirmeye yardımcı olur; komşu damar yapıları ve hava yoluyla ilişkisi de incelenebilir. Peritonsiller flegmon ile apse BT'de örtüşen morfolojik özellikler gösterebilir; kontrastlanan kenar tek başına bu ikisini güvenilir biçimde ayırt ettirmez. Parafarengeal yayılımın veya damar komplikasyonunun gözden kaçması, enfeksiyonun komşu derin boyun kompartımanlarına ilerlediğinin fark edilmesini engelleyebilir.

BT bulguları
- Palatin tonsilin büyümesi ve karşı tonsile göre belirgin asimetri
- Tonsil kapsülünün dışında, özellikle anterolateral peritonsiller alanda düşük atenüasyonlu odak
- Peritonsiller odağı çevreleyen kontrastlanan duvar; olgunlaşmamış koleksiyonda sınır silik olabilir
- Parafarengeal yağın çizgilenmesi veya silinmesi, enfeksiyonun tonsil yatağını aştığını düşündürür
- Tonsiller enfeksiyonlarda parafarengeal boşluğa yayılım görülebilir.
- Karotis arter ve juguler venin yer değiştirmesi, daralması veya çevresindeki inflamatuvar değişiklik
- Orofaringeal hava yolunda yana itilme veya daralma, derin boyun enfeksiyonlarında hava yolu basısı olasılığını düşündürür.
Normalde
Normal orofarenkste palatin tonsiller simetrik boyut ve benzer kontrastlanma gösterir; tonsil kapsülü çevresinde odaksal sıvı cebi bulunmaz. Tonsil enfeksiyonlarında görüntülemede apse yerleşimi ve parafarengeal uzanım değerlendirilir.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
