Özet

Akut bakteriyel servikal lenfadenit çocukta tek taraflı, hassas nod büyümesiyle ortaya çıkabilir ve bazı nodlar süpüre olarak apseleşir. BT yüzeyel ultrasonun kapsayamadığı derin nodal istasyonları, çevre boşlukları ve hava yolu ilişkisini gösterir; kontrastlı inceleme nekrotik merkezi canlı nodal dokudan ayırmaya yardımcı olur. Ancak halka tarzı kontrastlanma tek başına özgül değildir ve enfekte nodların bir bölümünde de görülebilir. Nodal enfeksiyonun komşu derin boşluğa uzanması veya damar/hava yolu etkisinin atlanması yayılımın olduğundan sınırlı yorumlanmasına yol açar.

Faz ve pencere

Venoz tanısal
Tek fazlı intravenöz kontrastlı boyun BT'sinde büyümüş oval nod, merkezî kontrastlanmayan düşük atenüasyon, çevresel nodal kontrastlanma ve perinodal yağ çizgilenmesi değerlendirilir; komşu nodlar ve derin boşluklar taranır.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Anjiyo (G 600 / M 150).

BT bulguları

  • Tek veya birden çok büyümüş, oval şekilli servikal lenf nodu
  • Nod içinde merkezî düşük atenüasyon ve çevrede kontrastlanan nodal doku
  • Nodal kapsül sınırının belirsizleşmesi ve perinodal yağda çizgilenme
  • Komşu nodlarda kümelenme veya birbirine yapışık nodal konturlar
  • BT, nekrotik nod morfolojisini değerlendirmenin yanı sıra, enfeksiyonun komşu derin boyun boşluklarına (parafarengeal, retrofarengeal) yayılımını ve hava yolu ilişkisini de ortaya koyar.
  • İnternal juguler ven veya karotis arteri çevreleyen ödem; damar lümeninde bası ya da tromboz şüphesi
  • Yüzeyel nodda deri altı ödemi ve cilde doğru koleksiyon uzanımı

Normalde

Normal servikal lenf nodları genellikle ovaldir ve komşu yağ planları ile nodal hilyum yapısı korunmuştur. Sağ ve sol aynı nodal seviyeleri karşılaştır; merkezî nekroz, kapsül dışına taşan yağ kirlenmesi ve damar kılıfına komşuluğu özellikle ara.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Reaktif lenfadenopati

Tüberküloz lenfadeniti

Kikuchi-Fujimoto hastalığı

Lenfoma
birden fazla nodal istasyonda homojen yumuşak doku nodları görülebilir; akut süpüratif yağ planı kirlenmesi daha az tipiktir.
Brankiyal yarık kisti enfeksiyonu
tipik nodal zincir dışında, belirli anatomik hat boyunca yerleşen kistik lezyondur.

Tuzaklar

  • Reaktif lenf nodlarında da halka tarzı kontrastlanma görülebilir; bu bulguyu tek başına nodal apse ölçütü sayma, merkezî sıvı ve çevre inflamasyonunu birlikte tarif et.
  • Halka şeklinde kontrastlanma süpüratif apse ile birlikte nekrotik metastatik lenf nodları veya tedavi sonrası değişikliklerde de görülebilir; kesin ayırım için klinik bağlam ve gerekirse doku örneklemesi gereklidir.
  • Nekrotizan nodal hastalık etkeni BT morfolojisinden kesinleştirilemez; görüntüyü klinik ve gerektiğinde doku örneklemesi yerine koyma.
