Özet

İleokolik invajinasyonda terminal ileum çekum ve kolona doğru teleskopik olarak ilerler; mezenter ve damarlar da içe çekilir. Tanıda ilk tercih ultrason olsa da atipik başvuruda veya başka nedenle çekilmiş BT'de bağırsak-bağırsak içine geçmesini ve olası komplikasyonları tanımak önemlidir. Kontrastlı BT, invajine segmentin duvar canlılığını, proksimal tıkanmayı ve olası öncü odağı aynı incelemede gösterebilir. İskemi veya perforasyonun atlanması bağırsak hasarının yaygınlığını olduğundan az gösterebilir.

Faz ve pencere

Portal tanısal
Portal venöz kontrastlı aksiyel ve koronal görüntülerde iç içe geçmiş bağırsak duvarları, arada taşınmış mezenterik yağ ve damarlar; duvar kontrastlanması, proksimal dilatasyon ve serbest sıvı birlikte değerlendirilir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Hedef işareti — aksiyel kesitte eşmerkezli bağırsak duvarı katmanları, ortadaki içeri çekilmiş segmenti çevreler.
  • Sosis görünümü — klinik muayenede hissedilen sosis şeklindeki kitleyi tanımlar; uzunlamasına kesitlerde uzamış, katmanlı bağırsak kitlesi ultrasonografide pseudokidney işareti olarak adlandırılır (S1, S5).
  • İçe çekilmiş mezenter — intussusceptum ile birlikte yağ ve damarlar kolon lümenine taşınır; damarlar kitle merkezine doğru uzanır.
  • İleoçekal yer değiştirme — terminal ileum duvarı çekum/çıkan kolon içinde izlenir ve normal ileoçekal birleşke seçilmesi güçleşebilir.
  • Proksimal obstrüksiyon — invajinasyon bağırsak tıkanıklığına yol açabilir; tıkanıklığın proksimalindeki ince bağırsak anslarında sıvı veya gaz birikimi görülebilir.
  • Duvar ödemi ve sıvı — invajine segmentte kalınlaşma, mezenterik ödem veya serbest sıvı eşlik edebilir.
  • İskemi şüphesi — invajine bağırsak duvarının kontrastlanması azalır ya da kaybolur; duvar içi gaz veya serbest hava ileri komplikasyonu düşündürür.

Normalde

Normalde terminal ileum çekumla birleşir, bağırsak duvarları tek sıra halinde seyreder ve çekum lümeninde içeri çekilmiş mezenter paketi bulunmaz. Aynı seviyede halka katmanlarının bağırsak duvarına ait olup olmadığını, mezenterik yağ ve damarların kolon içine taşınıp taşınmadığını kontrol et.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

İleoçekal Crohn hastalığı
terminal ileumda segmental duvar kalınlaşması ve çevre inflamasyon vardır; invajine mezenter paketi görülmez.

Tuzaklar

  • Aksiyel tek kesitte halka benzeri görünümü invajinasyon sayma; koronal planda uzamış segmenti ve içe taşınmış mezenteri doğrula.
  • İnvajinasyon içinde kalan mezenterik yağın lümen içi kitle gibi algılanabileceğini hatırla; yağın damarlarla birlikte içe çekildiği yöne bak.
  • Bağırsak duvarı kontrastlanmasını yalnız kalınlığa göre yorumlama; azalan kontrastlanma, serbest sıvı ve obstrüksiyonla birlikte iskemi açısından anlam kazanır.

    İleokolik invajinasyon. Kolon içindeki katmanlı bağırsak yapısı ve beraberinde çekilmiş mezenter ileokolik invajinasyonun anatomik karşılığıdır. Crohn segmental ileum kalınlaşması, adenit lenf düğümü kümeleri, volvulus ise burulmuş ve genişlemiş çekum/mezenter görünümü yapar.

  2. İnvajinasyon içindeki bağırsak duvarı komşu anslardan belirgin az kontrastlanıyor ve çevrede serbest sıvı var. En önemli endişe nedir?

    Bağırsak iskemisi. Azalmış duvar kontrastlanması ve serbest sıvı, mezenterik dolaşım bozulmasıyla gelişen iskemi olasılığını yükseltir. Lenfadenit lenf düğümleriyle sınırlı kalır; Meckel divertikülü öncü nokta olabilir ama bu perfüzyon bulgusunu açıklamaz; omental infarkt yağ dokusu odağıdır.
