Özet

Orta bağırsak volvulusu çoğunlukla malrotasyon zemininde ince bağırsak ve mezenterin dar kök çevresinde dönmesidir; çocukta ani safralı kusma ve karın ağrısı ile ortaya çıkabilir. Malrotasyon/volvulus kuşkusunda ilk değerlendirmede ultrason ve üst gastrointestinal seri kullanılır; BT rutin ilk inceleme değildir. BT zaten elde edilmişse SMA çevresinde ince bağırsak ve SMV’nin dönüşü ile bağırsak duvarı kontrastlanması değerlendirilir. Girdap işareti dönmeyi destekler; duvar kontrastlanmasında azalma iskemi olasılığını artırır. Görüntüleme klinik acil değerlendirmeyi geciktirmemelidir.

BT bulguları

  • Girdap işareti — SMV, mezenterik yağ ve ince bağırsak damarları SMA çevresinde spiral biçimde sarılır.
  • Mezenter damar dallarında yön değişikliği ve birbirine yaklaşan radyal damar yapıları dönmüş mezenterik pedikülü belirginleştirir.
  • Proksimal duodenum ve jejunumda dilatasyon, dönme düzeyinde ani kalibrasyon değişikliği ve distal bağırsaklarda kollaps görülebilir.
  • Kapalı ans görünümüne mezenterik konjesyon ve serbest sıvı eşlik edebilir; bu bulgular bağırsak iskemisi kuşkusunu artırabilir.
  • Bağırsak kan akımı bozulduğunda duvar kalınlaşması ve asit eşlik edebilir; duvar kontrastlanmasının azalması bağırsak iskemisini düşündürür.
  • Bağırsak duvarında gaz ve perforasyonu düşündüren serbest hava varlığı ayrıca değerlendirilmelidir.
  • Eşlik eden malrotasyon bulguları arasında duodenojejunal bileşkenin anormal konumu ve sağ ağırlıklı proksimal ince bağırsak yerleşimi bulunabilir.

Normalde

Normal mezenterik kökte SMV, SMA çevresinde spiral yapmaz; ince bağırsak damarları kökten anslara düzenli ve dallanan seyir gösterir. İskemi bulgusu olmayan volvulus olgusunda bağırsak mukozal kontrastlanması korunabilir; şüpheli ansı komşu ansların kontrastlanmasıyla karşılaştırın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

İnce bağırsak adezyon obstrüksiyonunda geçiş noktası ve dilate anslar görülebilir; mezenterik girdap farklı mekanik obstrüksiyonlarda da görülebildiğinden tek başına orta bağırsak volvulusunu kanıtlamaz.
İntussusepsiyon ultrasonografide hedef işaretiyle görülebilir; midgut volvulusla görüntüleme benzerlikleri olabileceğinden ayırıcı tanıda tutulmalıdır.
İnce bağırsak kapalı ans obstrüksiyonunda mezenterik girdap da görülebilir; girdap görünümü tek başına orta bağırsak volvulusunu kanıtlamaz.
Mezenterik ven trombozu, orta bağırsak volvulusu olgularında eşlik eden bir durum olarak bildirilmiştir.

Tuzaklar

  • SMV-SMA ilişkisindeki anormallik tek başına orta bağırsak volvulusunu göstermez; gelişimsel karşı yönlü damar sarılması normal varyant olabilir ve volvulusun girdap işaretinden ayırt edilmelidir.
  • Girdap işaretinin yönü ve görünürlüğü değişebilir; damarların aksiyel kesitte tek bir ilişkisinden çok ardışık kesitlerde devamlı spiral seyrine odaklanın.
  • Kontrastlı BT'de bağırsak duvarı kontrastlanmasının belirgin azalması veya kaybı bağırsak iskemisini düşündürür.
  • Erken volvulusta belirgin duvar kalınlaşması veya pnömatozis olmayabilir; bu bulguların yokluğu dönmeyi dışlamaz.
