Özet

Hashimoto tiroiditi, tiroid parankiminde kronik otoimmün inflamasyon ile seyreden ve zaman içinde değişken bez büyümesi gösteren bir hastalıktır. BT'de diffüz düşük atenüasyon, heterojenite, lobüle kontur ve değişken büyüme görülebilir; bu bulgular özgül değildir ve tanı laboratuvar ile klinik veriye dayanır. BT'nin rolü çoğunlukla başka nedenle çekilmiş incelemede yaygın bez değişikliğini fark etmek ve eşlik eden nodül, lenf nodu ya da bası etkisini göstermektir. Fokal psödolezyonun gerçek nodül veya lenfoma sanılması gereksiz endişeye; Hashimoto zeminindeki belirgin kitle ya da patolojik nodun otomatik olarak tiroidite bağlanması ise önemli ek hastalığın atlanmasına yol açabilir.

Faz ve pencere

Kontrastsız
Tiroidin normal iyot içeriğine bağlı görece yüksek atenüasyonuna kıyasla diffüz azalmış veya yamalı heterojen atenüasyon görülebilir; tek başına bu görünüm Hashimoto tanısı koydurmaz.
Venöz
Kontrastlı BT'de büyüyen bezde heterojen veya azalmış parankimal kontrastlanma ve lobüle kontur izlenebilir; bazı olgularda görüntüleme oldukça hafif bulgu verebilir. Fokal gerçek kitle, tiroid dışına uzanım ve servikal lenf nodları eşzamanlı değerlendirilir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Mediasten (G 350 / M 50).

BT bulguları

  • Her iki tiroid lobunda diffüz büyüme görülebilir.
  • Normal tiroidin görece yüksek atenüasyonuna kıyasla diffüz düşük atenüasyon.
  • Kontrastsız ya da kontrastlı seride yamalı/heterojen parankim görünümü ve lobüle dış kontur.
  • Bez içinde gerçek kapsüllü nodül oluşturmayan, sınırları belirsiz coğrafi psödolezyon benzeri alanlar.
  • Büyüyen bezin komşu yapılara (örn. trakea) hafif bası etkisi yapabileceği belirtilmiştir.
  • Belirgin fokal solid kitle veya patolojik servikal lenf nodu varsa eşlik eden tümör olasılığını bağımsız olarak araştır.

Normalde

Normal tiroid, krikoid-altı ve üst trakea düzeyinde iki lobu ile istmusu seçilen, görece yüksek atenüasyonlu, düzgün konturlu ve kontrastlı incelemede belirgin kontrastlanma gösteren bir yapıdır. Hashimoto şüphesinde her iki lobun boyut ve atenüasyon dağılımını, kontur lobülasyonunu ve karşılaştırmalı yağ planlarını incele; BT görünümünün normal varyasyon ve diğer tiroiditlerle örtüşebildiğini akılda tut.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Graves hastalığı ve diffüz hiperplazi de diffüz bez genişlemesi ile seyrebilir. Hashimoto’da düşük atenüasyon ve heterojen kontrastlanma daha sık görülebilir; BT paternleri kesin ayrım sağlamaz, klinik ve biyokimyasal verilerle eşleştirilir.
Subakut granülomatöz tiroidit (de Quervain) ve akut süpüratif tiroidit de difüz tiroid hastalıkları listesindedir ve ayırıcı tanı için klinik değerlendirme gereklidir.
Akut süpüratif tiroidit
BT'de görülen büyük apse veya intratiroidal sıvı koleksiyonu, Hashimoto'nun diffüz paterninden ayrılır.
Tiroid lenfoması
Hashimoto zemininde gelişebilen fokal veya infiltratif kitle ve patolojik lenf nodları sıradan diffüz heterojenlikten ayrılmalıdır.
Anaplastik tiroid karsinomu
kapsül dışına yayılım, komşu yapılara tutulum ve düzensiz nekrotik kitle ile belirgin ayrıştırıcı özellikler gösterir.

Tuzaklar

  • Tiroid atenüasyonu iyot düzeyi ve kontrast kullanımından etkilenir; tek bir HU değerini Hashimoto için tanı eşiği kabul etme.
  • BT'de görülen yamalı/heterojen parankim değişiklikleri nonspektif olup, gerçek nodüllerden ayrımı için histopatolojik veya ultrasonografi karşılığı gerekebilir.
  • Hashimoto ile lenfoma ve papiller kanser birlikteliği bildirilmiştir; yeni fokal kitleyi yalnız bilinen tiroidite bağlayıp geçme.
  • Heterojen kontrastlanma tek başına tiroidit türünü göstermez; görüntüleme bulgusunu seroloji ve ultrason karşılığından ayır.
