Özet

Viral sialadenit en sık parotisi tutar; kabakulakta şişlik iki tarafta eşzamanlı olmayabilir ve tanı çoğunlukla klinik değerlendirmeyle konur. BT, tipik viral parotiti doğrulamak için rutin inceleme değildir; atipik tek taraflı şişlikte taş, derin boyun yayılımı veya apse gibi alternatifleri araştırmaya yardım eder. Bez hacmi artışı ve çevre yağ planlarındaki silinme görüntüde görülebilir, ancak bunlar virüse özgü değildir. Görüntüleme bulgularına tek başına bakarak viral etkeni kesin olarak belirtmek, bakteriyel enfeksiyon veya kitleyi gözden kaçırmaya yol açabilir.

Normalde

Skull bazadan hiyoid kemiğe kadar alınan ince dilimli aksiyel BT kesitlerinde parotis bezleri karşılaştırılırken; bez hacmi, parankim homojenliği ve çevre fasyal planlardaki yağın bütünlüğü değerlendirilir. Aynı seviyedeki aksiyel BT kesitlerinde sağ ve sol parotis bezleri karşılaştırılır; bez hacmi artışı, parankimdeki heterojenite ve çevre yağ planlarındaki infiltrasyon ('dirty fat' görünümü) incelenir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • İki taraflı parotis büyümesini tek başına kabakulak olarak adlandırma; IgG4 ilişkili hastalık ve Sjögren de bezleri birlikte büyütebilir.
  • Viral sialadenit genellikle iki taraflı tutulum yapar; tek taraflı tutulum nadir de olsa ayırıcı tanıda düşünülmelidir.
  • Çevresel ödem ile apse duvarını karıştırma; apse için bez içinde sıvı atenüasyonlu merkez ve çevresel kontrastlanma birlikte aranır.
