Özet

Fungal top, çoğunlukla bağışıklık sistemi normal kişilerde görülen, tek sinüs boşluğunda sınırlı kalan noninvaziv yoğun hif birikimidir; en sık maksiller sinüs tutulur. Kontrastsız BT'de tek sinüs opasifikasyonu içindeki hiperdens veya noktasal kalsifiye odakları ve çevre kemikte kronik osteit bulgularını birlikte gösterir. BT fungal topu düşündüren bulguları gösterebilir; doku invazyonunun dışlanması ve etkenin tanımlanması için histopatolojik ve mikrobiyolojik inceleme gerekir. Fungal top ile invaziv hastalığın ayrımında zamanında görüntüleme ve invaziv yayılım bulgularının değerlendirilmesi önemlidir.

Faz ve pencere

Kontrastsız tanısal
Tek sinüs içinde yumuşak doku yoğunluklu opasifikasyon ve merkezî/dağınık hiperdens ya da kalsifiye odaklar aranır; aynı seride sinüs duvarı sklerozu, kalınlaşması ve fokal erozyon kemik algoritmasında değerlendirilir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).

BT bulguları

  • Genellikle tek paranazal sinüsün yuvarlak, oval veya lümeni dolduran heterojen opasifikasyonu.
  • Opak içerik içinde belirgin hiperdens odaklar; bunlar kaba, noktasal ya da kümelenmiş kalsifikasyon biçiminde olabilir.
  • Hiperdens materyalin çevresinde daha düşük yoğunluklu sekresyon veya mukozal kalınlaşma bulunabilir.
  • Komşu sinüs duvarında skleroz ve kalınlaşma kronik kemik değişikliklerini destekler.
  • Uzun süreli basınçla duvar incelmesi, genişleme veya sınırlı fokal erozyon görülebilir; bu bulgu tek başına doku invazyonu demek değildir.
  • Premaxiller yağ infiltrasyonu, sinüs BT'sinde invaziv fungal sinüzitle bağımsız ilişkili bulunan bulgulardandır. Tanı, tek sinüste hiperdens intralüminal materyal ve kronik kemik değişiklikleri gibi bulgularla birlikte değerlendirilir.
  • Kontrast verilmiş incelemede fungal agregat kontrastlanmazken çevre inflamatuvar mukoza kontrastlanabilir; rutin tanı için kontrastsız inceleme yeterlidir.
  • MR'da fungal top, T2 ağırlıklı görüntülerde çevresindeki yüksek sinyalli hipertrofik mukozadan ayrılan düşük sinyalli alanlar gösterebilir.

Normalde

Fungal topta tutulan sinüste opasifikasyona intralüminal kalsifikasyonlar ve çevre duvarda hiperostoz eşlik edebilir. Hastalıklı tarafta opasifikasyonu, intralüminal kalsifikasyonları ve sinüs duvarındaki sklerozu değerlendir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Alerjik fungal rinosinüzitte genişleyici sinüs hastalığı ve kemik yeniden şekillenmesi görülebilir.
İnvaziv fungal rinosinüzitte sinüs dışı periantral veya premaxiller yağ infiltrasyonu görülebilir; MRI'da devitalize mukozada kontrastlanma kaybı bulgusu saptanabilir.
Bakteriyel sinüzit
lümen opasifikasyonu ve sıvı seviyesi yapabilir, fakat kümelenmiş intralüminal hiperdens/kalsifiye odaklar ile kronik skleroz daha az tipiktir.
Endodontik materyal veya yabancı cisim
maksiller sinüs içinde yoğun odak oluşturur; diş işlemi öyküsü ve lineer/dental materyal görünümü fungal agregat yerine geçebilir.
Odontojenik sinüzit
aynı taraftaki kök apeksi, sinüs tabanı defekti veya dental materyal odağı araştırılır; opasifikasyon tek başına fungal top değildir.
Alerjik fungal rinosinüzitte birden fazla sinüs tutulumu görülebilir; fungal top ise genellikle tek bir sinüsle sınırlıdır.

Tuzaklar

  • Sinüs duvarındaki sklerozu aktif invazyon olarak yorumlama; bu kronik osteit olabilir, sinüs dışı yumuşak doku yayılımı ayrı aranmalıdır.
  • Tek başına hiperdens odak fungal top için özgül değildir; dental dolgu, yabancı cisim veya yoğun sekresyon olasılığını diş kökleri ve işlem materyaliyle karşılaştır.
  • Fokal kemik erozyonu fungal topu dışlamaz; sinüs duvarı veya kafa tabanı erozyonu fungal top olgularında bildirilmiştir.
