Özet

Kronik rinosinüzit osteiti, uzun süreli mukozal hastalıkla ilişkili kemik yeniden yapılanması ve yeni kemik oluşumudur; BT’de en sık kalınlaşma ve skleroz şeklinde fark edilir. Kontrastsız ince kesit sinüs BT, hem kemik değişikliğini hem eşlik eden mukozal opasifikasyon ve drenaj yolu hastalığını birlikte gösterir. Kalın ve sklerotik duvar kronik hastalık yükünü destekler, ancak tek başına aktif enfeksiyonu veya belirli bir etkeni kanıtlamaz. Düzgün remodelingin destrüktif enfeksiyon ya da kitle gibi yorumlanması yanlış yönlendirebilir; fokal kortikal yıkım varsa yalnız kronik osteite bağlanmamalıdır.

Faz ve pencere

Kontrastsız tanısal
Kemik algoritmalı sinüs BT’de özellikle maksiller ve sfenoid sinüs duvarlarında kalınlaşma, skleroz ve trabeküler yeniden yapılanma; yumuşak doku penceresinde eşlik eden mukozal opasifikasyon görülür. Osteit tanımı için kontrastlı faz gerekli değildir.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Maksiller sinüs ön veya arka duvarında düzgün kalınlaşma ve artmış kemik yoğunluğu.
  • Sfenoid sinüs duvarında sklerotik yeniden yapılanma; kemik konturunun bütünlüğünün korunması.
  • Etmoid septalarda kronik kalınlaşma veya düzensiz yeniden şekillenme; ince lamellerde eşlik eden incelme de olabilir.
  • Aynı sinüste mukozal kalınlaşma ya da kısmi opasifikasyonla birlikte kemik duvarı değişikliği.
  • Birden fazla sinüs duvarında yaygın tutulum ve kronik drenaj yolu daralması.
  • Yumuşak doku kitlesi, agresif periostal reaksiyon veya fokal korteks kesintisinin bulunmaması, kronik remodeling lehinedir.

Normalde

Normal maksiller ve sfenoid sinüs duvarları ince, düzgün ve kortikal sınırları belirgindir; sinüs içi hava korunur. Aynı kesitte kontralateral sinüs duvarının kalınlığını ve yoğunluğunu kıyaslayıp kemik değişikliğinin mukozal hastalıkla aynı sinüste örtüşüp örtüşmediğine bakın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Fibröz displazi
kemik segmentini genişleten buzlu cam matriksli medüller lezyon yapar; izole duvar kalınlaşması değildir.
Ossifiye fibrom
keskin sınırlı, ekspansil ve yoğunluk değişkenliği gösteren odak olarak normal kemikten ayrılır.
Paget hastalığı
daha yaygın kraniofasiyal kemik kalınlaşması ve kaba trabeküler yapı oluşturabilir.
Mukosel
sinüs boşluğunu genişletip duvarı inceltebilir veya düzgünce yeniden şekillendirebilir; sklerotik duvar kalınlaşması temel bulgu değildir.

Tuzaklar

  • Sinüs duvarı kalınlığını tek başına kronik rinosinüzit kanıtı saymayın; normal ve az hastalıklı etmoidlerde de kemik değişiklikleri görülebilir.
  • Etmoid kemik lamelleri ince olduğundan remodelingi osteit sanmak kolaydır; kalınlaşma ve sklerozu özellikle maksiller veya sfenoid duvarla birlikte değerlendirin.
  • Düzgün sklerozu agresif kemik yıkımıyla eşitlemeyin; korteks devamlılığı ve sinüs dışı yumuşak doku yayılımı ayrı bulgulardır.
