Karın duvarı apsesi, cilt altı, fasya veya kas içinde sınırlı pürülan koleksiyondur; cerrahi, enjeksiyon, yara ya da komşu intraabdominal enfeksiyon sonrası gelişebilir. IV kontrastlı BT, sıvı merkezini çevresindeki kontrastlanan duvardan ayırır ve koleksiyonun hangi karın duvarı katmanında olduğunu gösterir. Aynı inceleme olası yabancı cisim, gaz, fistül ve peritoneal ya da retroperitoneal kaynakla bağlantıyı araştırır. Derin uzanım veya altta yatan kaynak atlanırsa enfeksiyonun yaygınlığı ve tekrarlama nedeni eksik tanımlanabilir.

- Kontrastsız
- IV kontrast verilemiyorsa koleksiyonun sıvı atenüasyonu, gazı ve yabancı cisim/kalsifikasyon ilişkisi görülebilir; çevresel duvar kontrastlanması değerlendirilemediğinden flegmon ile apse ayrımı kısıtlıdır.

BT bulguları
- Cilt altı yağ içinde, yüzeyel fasyaya komşu veya kas içinde yerleşen, sınırları belirgin sıvı yoğunluklu koleksiyon.
- Koleksiyon periferinde düzgün ya da düzensiz kontrastlanan kapsül; merkezde kontrastlanmayan sıvı içeriği.
- Koleksiyon çevresinde cilt kalınlaşması, subkutan yağ çizgilenmesi ve yüzeyel fasya kalınlaşması.
- Koleksiyon içinde gaz kabarcıkları veya sıvı-gaz seviyesi; yakın zamanda girişim yoksa enfeksiyonu destekleyen ancak tek başına özgül olmayan bulgu.
- Cerrahi sütür, mesh, dren, enjeksiyon materyali veya diğer yabancı cismin koleksiyonla doğrudan ilişkisi.
- Derin fasya, rektus kılıfı, karın içi veya retroperitoneal boşluğa uzanan fistül ya da inflamatuvar trakt.
- Komşu apandisit, divertikülit, bağırsak anastomozu kaçağı veya osteomiyelit gibi olası enfeksiyon kaynağı.
Normalde
Normal karın duvarında cilt altı yağ lobülleri, yüzeyel ve derin fasya planları ile rektus ve oblik kaslar düzenli katmanlar halinde, fokal sıvı koleksiyonu olmadan izlenir. Sağlam kaslar simetrik kalınlıkta ve homojen atenüasyondadır; karın içi organlarla cilt altı arasında fistül traktı bulunmaz. Kıyasta koleksiyonun hangi katmanda başladığını, duvar kontrastlanmasını ve komşu normal fasya-kas sınırlarının silinmesini izleyin.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

- Selülit/flegmon
- cilt altı yağ ve fasya yaygın çizgilenir ancak merkezinde sınırlı sıvı koleksiyonu ve kapsüllü çevresel duvar kontrastlanması yoktur.
- Enfekte hematom
- çoğunlukla kas veya cerrahi yatakta kan ürünleri ve değişken atenüasyon gösterir; klinik zamanda göre kontrastlanan duvar gelişebilir, önceki kanama/girişim bilgisi önemlidir.
- Seroma
- cerrahi yatakta homojen sıvı yoğunluklu, ince duvarlı koleksiyondur; belirgin çevresel inflamasyon, kalın kontrastlanan duvar veya gaz yoksa enfeksiyon daha az olasıdır.
- Kistik/nekrotik tümör
- nadiren karın duvarı apsesiyle birlikte bulunabilir; apse görünümü altta yatan tümörü dışlamaz.
- Karın duvarı endometriozisi
- sıklıkla cerrahi skarla ilişkili solid yumuşak doku nodülüdür; yalnız sıvı merkezi ve inflamatuvar kapsülle sınırlı değildir.
Tuzaklar
- Erken flegmon rim kontrastlanan apseyi taklit edebilir; gerçek koleksiyonda çevresel duvarla ayrılan sıvı merkezinin bulunduğunu farklı planlarda doğrulayın.
- Ameliyat sonrası küçük gaz odakları enfeksiyon kanıtı değildir; işlem zamanı, gazın dağılımı ve çevresindeki koleksiyon/yağ inflamasyonunu birlikte okuyun.
- Mesh çevresindeki sıvı enfeksiyon, seroma veya beklenen ameliyat sonrası değişiklik olabilir; mesh boyunca uzanan rim, gaz ve çevresel inflamasyonu arayın.
- Koleksiyonun derin uzanımı ve komşu yapılarla ilişkisi, çoklu düzlem rekonstrüksiyonlar (koronal ve sagittal) kullanılarak daha güvenilir izlenebilir.
