Özet

Diyaframdan köken alan primer tümörler nadirdir; kist ve lipom gibi benign lezyonların yanında sarkomlar görülebilir, metastaz veya komşu organ tümörünün doğrudan uzanımı da diyaframı tutabilir. BT, lezyonun yağ, sıvı veya yumuşak doku bileşenini ve toraks-abdomen ilişkisini gösterir; ince diyafram nedeniyle kökeni her zaman kesinleştiremez. Lezyon merkezinin kas ya da tendon içinde olması, komşu akciğer, plevra, karaciğer ve adrenal yapıların ayrı izlenmesi tanısal yönlendirmeyi sağlar. Komşu kitle yalnızca diyaframa dayanıyorsa BT ile invazyon kesin denemez; aşırı kesin köken veya yayılım ifadesi cerrahi planı yanıltabilir.

Faz ve pencere

Kontrastsız: Makroskopik yağ içeriği, kalsifikasyon ve lezyonun kas ya da tendonla ilişkisi değerlendirilir; homojen yağ dansiteli lipom olasılığını destekler, ancak maligniteyi dışlamak için tek başına yeterli değildir.
Portal tanısal: Kontrastlanan yumuşak doku kitlesinin diyafram kası veya santral tendona merkezlenmesi, nekrozlu alanları ve toraks-abdomen kompartımanlarına uzanımı izlenir; komşu organla yalnız temas invazyon kanıtı sayılmaz.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Mediasten (G 350 / M 50), Batın (G 400 / M 50).

Normalde

Normal diyafram, toraks ile abdomeni ayıran ince, kubbe biçimli kas-tendon yapısıdır. Şüpheli alanda kas liflerinin konturunu ardışık aksiyel ve koronal kesitlerde takip et; lezyon merkezinin diyafram içinde mi, yoksa plevra, akciğer, karaciğer veya adrenal organda mı olduğunu kıyasla.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Komşu organ tümörünün doğrudan uzanımı: akciğer, plevra, karaciğer veya adrenal kitleye kesintisiz devamlılık varsa sekonder tutulum düşünülür; yalnız temas invazyonu kanıtlamaz.
Diyafram metastazı: bilinen primer tümör, birden çok serozal nodül veya başka uzak odaklar eşlik edebilir; izole kitlede görüntüleme kökeni kesinleştirmeyebilir.
Komşu organ kitleleri veya enfeksiyon odakları diyaframı ikincil tutabilir veya benzer görüntüleyici bulgular verebilir; kökenin kesin belirlenmesi için anatomik sınırların ayrı ayrı izlenmesi gerekir.
Diyafram lipomu: homojen makroskopik yağ içeriği ve belirgin nodüler yumuşak doku komponenti olmaması benign yağlı lezyonu destekler.
Eventrasyon veya diyafram invajinasyonu: kitle yerine düzgün kubbeleşme ya da diyaframın kıvrılmış konturu izlenir; ayrı büyüyen yumuşak doku odağı bulunmaz.
Soliter fibröz tümör (plevra kökenli): plevra yüzeyinde kitle oluşturur; diyaframın kas-tendon hattı boyunca merkezlenme ve iki kompartımana uzanma göstermez.

Tuzaklar

Diyafram ince olduğundan kitle ona dayanırken kökeni belirsiz kalabilir; organ sınırlarını kesit kesit izleyip yalnız temas varsa invazyon tanısı koyma.
Yağlı kitlede ince septalar görülebilir; kalınlaşmış septa veya nodüler solid komponent varsa basit lipom olarak kesin sınıflama yapma.
Periferik kas lifleri ve normal diyafram slipleri fokal kalınlaşma taklidi yapabilir; beklenen tutunma yönünü ve karşı tarafı incele.
