Bilinen malignitesi olan hastada yeni fokal kostal ağrı veya evreleme BT’sinde kemik odağı, kaburga metastazı açısından incelenir. Kemik metastazları litik veya sklerotik olabilir; BT kemik yıkımını, kortikal kesintiyi ve patolojik kırığı değerlendirmeye, MR ise yumuşak doku uzanımını göstermeye yardımcı olur. Kaburga metastazı zemininde patolojik kırık görülebilir; BT dâhil görüntüleme lezyonu ve kırığı gösterebilir.
Faz ve pencere
- Kontrastsız tanısal
- Kemik penceresinde kaburga boyunca litik kemik kaybı, sklerotik yoğunluk artışı veya ikisinin karışımı; kortikal kesinti, kırık hattı ve periostal değişiklik aranır. Kontrastsız seri lezyonun doğal atenüasyonunu göstermeye yardımcı olur; metastazı tanımlayan evrensel tek bir HU eşiği yoktur.

- Kortikal tutulum — endosteal çentiklenme, kortikal yeniden şekillenme/kalınlaşma veya kortikal kesinti şeklinde görülebilir; fokal incelme tek başına metastaza özgü değildir.
- Ekstraosseöz yumuşak doku komponenti — kaburga korteksinin dışına uzanan yumuşak doku kitlesidir; yalnız plevral temas bu bulguyu tanımlamaz.

- Osteomiyelit
- enfeksiyon da kortikal tutulum, periostal reaksiyon veya yumuşak doku değişikliği yapabilir; bu bulgular tek başına metastaza özgü değildir.
- Bilinen malignitesi olan hastada tek kaburgadaki kırık metastazla ilişkili olmayabilir; travma olmaksızın gelişen kaburga kırığında primer tümör de düşünülmelidir.
Tuzaklar
- Kalluslu kırık metastazı taklit edebilir; ince kesitlerde kırık hattını, düzenli periostal iyileşmeyi ve önceki tetkiklerdeki değişimi birlikte inceleyin.
- Tedavi sonrası metastaz sklerotikleşebilir; bu değişiklik yanıtla ilişkili olabilse de tek başına yanıtı veya progresyonu kanıtlamaz ve diğer bulgularla birlikte değerlendirilir.
