Özet

Konstriktif (obliteratif) bronşiolit, bronşiol çevresindeki fibrozis ve lümen daralmasıyla karakterize progresif bir küçük hava yolu sürecidir. BT’de sentrilobüler nodüller, dallanan opasiteler, bronş duvarı değişiklikleri, mozaik atenüasyon ve ekspiratuvar hava hapsi görülebilir; bulgular örtüşür ve histolojik alt tipi tek başına güvenilir biçimde belirlemez. İnspirasyon fazı ince kesit BT bronşiyollerin ve parankimal yapıların incelenmesine olanak tanır; eş düzey ekspirasyon görüntüleri ise küçük hava yolu obstrüksiyonuna bağlı bölgesel hava hapsinin değerlendirilmesi için gereklidir. Rapor, görüntüleme örüntüsünü tarif etmeli; histoloji ve klinik bağlam olmadan alt tip atamamalıdır.

Faz ve pencere

Kontrastsız HRCT tanısal
İnspirasyon ince kesit akciğer görüntülerinde sentrilobüler nodüller, tree-in-bud, bronş duvarı kalınlaşması, bronşektazi ve eşlik eden parankimal değişiklikleri değerlendirin. Bu bulgular foliküler ve konstriktif bronşiolit arasında tek başına kesin ayrım sağlamaz.
Ekspirasyon HRCT tanısal
Eş düzey inspirasyon-ekspirasyon görüntülerinde bölgesel hava hapsi ve mozaik atenüasyonu arayın; hava hapsi küçük hava yolu disfonksiyonunu destekler, ancak histolojik alt tipi veya nedeni belirlemez.

Önerilen pencereler: Akciğer (G 1500 / M -600).

BT bulguları

  • Sentrilobüler nodüller — bronşiol merkezli nodüler opasiteler hücresel veya foliküler bronşiolite eşlik ettiğinde görülebilir, ancak özgül değildir.
  • Tree-in-bud — küçük hava yollarındaki dallanan dolum veya inflamasyon görünümüdür; enfeksiyon ve aspirasyon gibi başka nedenlerde de oluşur.
  • Ekspiratuvar hava hapsi — konstriktif bronşiolitte küçük hava yolu daralmasıyla birlikte görülebilir; tek başına histolojik tanı koydurmaz.
  • Mozaik atenüasyon — küçük hava yolu obstrüksiyonuyla ilişkili bulunabilir ve akciğer hastalıklarında sık görülen bir bulgu olarak raporlanır.
  • Bronşektazi, buzlu cam veya retikülasyon — altta yatan sistemik hastalık ve eşlik eden parankimal süreçlere göre değişebilir.
  • BT ile histoloji arasındaki örtüşme — aynı görüntüleme paterni farklı bronşioler patolojilerde görülebildiğinden biyopsi veya klinik bilgiler olmadan alt tip kesinleştirilmez.

Ölçütler ve sınıflamalar

Foliküler bronşiolit
Bronşiol çevresinde reaktif lenfoid folliküllerle tanımlanır. BT’de sentrilobüler nodüller, bronş duvarı değişikliği veya bronşektazi görülebilir; bulgular altta yatan hastalığa göre değişir.
Konstriktif (obliteratif) bronşiolit
Bronşiol çevresindeki konsantrik fibrozis lümeni daraltır veya obliterasyona götürür. Ekspirasyonda mozaik atenüasyon ve hava hapsi gözlenebilir; ancak spesifik histolojik alt tip için klinik öykü ve doku incelemesi gereklidir.
Proliferatif bronşiolitis obliterans
Lümen içi fibroinflamatuvar polipoid tıkaçlarla tanımlanan proliferasif bronşiolitis obliterans, konstriktif bronşiolitis obliterans ile aynı histolojik süreç değildir; ancak BT bulguları örtüştüğü için alt tipi tek başına kesin belirlemekte yetersiz kalır.

Normalde

Normal küçük hava yolları çoğu BT kesitinde doğrudan seçilemez; belirgin sentrilobüler nodül, tree-in-bud, bronş duvarı değişikliği veya ekspiratuvar lobüler hava hapsi yoktur. Bu işaretlerden biri bulunduğunda dağılımı ve eşlik eden bulguları tarif edin, fakat BT görünümünü tek başına foliküler ya da konstriktif histoloji olarak adlandırmayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Mozaik atenüasyonun ayırıcı tanımında, ekspirasyon görüntülerindeki hava hapsi varlığı hava yolu obstrüksiyonunu; damar kalibresindeki azalma ve kronik vasküler değişiklikler ise vasküler etiolojiyi destekler.

Tuzaklar
