Özet

Ampiyem necessitans, plevral ampiyemin parietal plevrayı aşarak göğüs duvarı yumuşak dokularına fistülize olmasıdır; tüberküloz sık bir nedendir, ancak aktinomikoz ve diğer enfeksiyonlar da benzer yayılım yapabilir. Kontrastlı toraks BT, plevral ampiyemin göğüs duvarına uzanımını ve komşu kaburgadaki yıkım gibi eşlik eden bulguları gösterebilir; akciğer parankiminin de kapsamlı değerlendirilmesini sağlar. Plevral bağlantı tanınmazsa göğüs duvarındaki kitle yumuşak doku kitlesi veya malignite olarak yanlış yorumlanabilir. Görüntüleme enfeksiyon etkenini tek başına belirlemez; koleksiyonun örneklenmesi gerekebilir.

İyileşme sonrasında rezidüel plevral kalınlaşma ve kalsifikasyon gelişebilir; ampiyem necessitans olgularında komşu kaburgada yıkım da görülebilir.

BT bulguları

  • Plevra-göğüs duvarı devamlılığı — plevral sıvı koleksiyonu ile interkostal veya kas içi koleksiyonun aynı odak boyunca birleşmesi.
  • Parietal plevra geçişi — koleksiyonun plevra sınırını aşıp ekstraplevral yağ ve interkostal kas içine uzanması.
  • Tüberküloz ampiyeminde lokülasyonla birlikte plevral kalınlaşma ve kontrastlanma görülebilir.
  • Plevral enfeksiyon, parietal plevrayı aşarak komşu göğüs duvarı ve çevre yumuşak doku yapılarına doğru ilerleyebilir.
  • Göğüs duvarı koleksiyonu, çevresel kontrastlanan sıvı atenüasyonlu bir yapı şeklinde görülebilir.
  • Komşu kaburga değişikliği — kronik enfeksiyonla kortikal düzensizlik, erozyon veya skleroz gelişebilir.
  • Ampiyemle ilişkili bronkoplevral fistül, BT ile araştırılabilecek komplikasyonlardandır.
  • Göğüs duvarına uzanan ampiyem, plevral koleksiyonla iletişim gösteren kistik bir göğüs duvarı lezyonu şeklinde izlenebilir.

Normalde

Normalde plevra boşluğu her iki hemitoraksta yaklaşık 10 mL sıvı içerir. Plevral sıvı ve plevral hastalığı değerlendirirken BT, plevra ile akciğer parankiminin kapsamlı incelenmesini sağlar.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Göğüs duvarı apsesi
cilt veya kas içi odak olabilir; plevral koleksiyonla doğrudan devamlılık yoksa ampiyem necessitans desteklenmez.
Toraks aktinomikozu, maligniteyi taklit edebilen ve ampiyem necessitansla ilişkili olabilen bir enfeksiyondur.
Nekrotik plevral tümör, göğüs duvarı kitlesinin ayırıcı tanısında düşünülebilir; kontrastlı toraks BT, malign yumuşak doku kitleleri gibi ayırıcıları değerlendirmeye yardımcı olur.
İşlem sonrası fistül
koleksiyon eski torakostomi veya cerrahi hattını izleyebilir; öykü ve giriş yeriyle anatomik uyum aranır.
Subfrenik koleksiyon
diyafram altından yukarı uzanır; plevra boşluğuyla bağlantı yerine abdominal kaynak veya diyafram geçişi görülür.

Tuzaklar

  • Göğüs duvarı kitlesi ampiyem necessitansın sunumu olabilir; BT'de göğüs duvarı koleksiyonu ile plevral sıvı arasındaki devamlılığı değerlendirin.
  • Kronik ampiyem duvarı yoğun kalsifiye olabilir; kemik penceresinde plevral kalsifikasyonu kaburga korteksinden ayırın.
  • Plevral hava tek başına bronkoplevral fistül kanıtı değildir; yakın zamanda drenaj/işlem olup olmadığını ve hava ile bronş arasında görünür ileti bulunup bulunmadığını kontrol edin.
