Özet

Hidrosalpinks, tuba uterinanın sıvı ile genişlemesidir ve çoğunlukla distal tıkanıklık ya da geçirilmiş inflamasyonla ilişkilidir; piyosalpinks ise tüp içindeki enfekte pürülan içeriği gösterir. BT'de pelvik inflamatuvar hastalıkta pelvik yağ çizgilenmesi ve fasiyal planlarda silikleşme görülebilir; hidrosalpinks ve piyosalpinks de pelvik inflamatuvar hastalık kapsamında değerlendirilen tubal bulgulardır. Piyosalpinksin erken tanınması pelvik inflamasyonun yayılımını ve tubo-ovaryan apseye ilerleme olasılığını anlamaya yardım eder. Tübüler lezyonu over kisti sanmak, apse odağını veya tuba torsiyonu gibi acil eşlikçileri gözden kaçırabilir.

Portal tanısal
Görüntülemede hidrosalpinks genişlemiş tübüler adneksiyel yapı, piyosalpinks ise kompleks veya ekojenik intratubal içerik ve kalınlaşmış, hiperemik ya da kontrastlanan tubal duvar ile uyumlu olabilir. Hidrosalpinks ve piyosalpinks, fallop tüpünü etkileyen ve farklı görüntüleme yöntemleriyle ayırt edilebilen durumlardır.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Genişlemiş fallop tüpü kontrastlı BT'de kıvrımlı tübüler yapı olarak izlenir.
  • Hidrosalpinks, distal tubal tıkanıklığa bağlı olarak tuba lümeninde seröz sıvı birikmesiyle oluşur.
  • Kalınlaşmış, hiperemik ve/veya kontrastlanan tubal duvar salpingit için bir görüntüleme bulgusudur.
  • Kompleks tubal lümen — heterojen veya ekojenik intratubal içerik piyosalpinksle uyumludur.
  • Çevre pelvik yağ çizgilenmesi — pelvik inflamatuvar hastalıkta pelvik yağ dokusunda çizgilenme ve fasiyal planlarda silikleşme görülebilir.
  • Uterus ve over komşuluğu — tüpün uterin kornu ile adneks arasında izlenmesi tubal kaynağı gösterir; over ayrı seçilebilir.
  • Koleksiyon veya apseye geçiş — tubo-ovaryan apse, multiloküle adneksiyel kitle şeklinde görülebilir.

Normalde

Tuba uterina hastalıklarının değerlendirilmesinde ultrason, BT ve MR gibi görüntüleme yöntemleri kullanılır. Şüpheli yapıda duvar kalınlığı, lümen genişlemesi ve çevre yağ planlarındaki değişiklikler değerlendirilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
